Management lied about ordering one pilot only in the wheelhouse. Doctor lied about pilot's health. Pilot pleaded guilty & sentenced to 18 months. Ferry director sentenced to 366 days. 11 dead. 70 injured. $90 million in lawsuits.

I was living in Brooklyn at the time. It was during the ALCS too. I went down for a walk that afternoon after class to listen to sports radio and take my dog out for a walk. I went down to the 69th Street Pier and I remember having no idea what was happening not that far across the water from where I was. I remember one woman had her legs torn off from the impact and she lingered on for several months in an induced coma, but sadly she didn't make it. I've been on that ferry too. It never seems dangerous either. So nice on a hot summer night.

From Wikipedia: On March 8, 2005, the National Transportation Safety Board (NTSB) published a report on its investigation into the incident. The NTSB determined the probable cause of the collision was the assistant pilot's unexplained sudden incapacitation, with a contributory cause of the Master's failure to maintain command and control of his vessel.[2] 60 Minutes uncovered that the assistant pilot was short on sleep when he crashed the boat. The assistant pilot tried to commit suicide after the crash and admitted he had passed out on painkillers in the boat's pilothouse. He later pleaded guilty to 11 counts of seaman's manslaughter. The ferry director also pleaded guilty after failing to enforce a rule requiring that ferries be operated by two pilots.[3]

The captain under control by himself in the staten island side passed out because he did not report a previous medical condition he had to the ferry company which made him pass out. So the fiery was left drifting towards the maintenance pier.

look at the photos, the cement pier sliced through the first passenger deck for more than half the length of the vessel like a giant guillotine... these weren't impact injuries they were eviscerations....
