Having the crank at the end of the large grip with some type of driveshaft to the front so that you could operate in confined areas would be nice.

There are some normal looking rachets that have a driveshaft inside. Put the socket over the fastener and turn clockwise the plastic handle overmolding to crank the socket

As mentioned by @VictorGarciaR, check out the FACOM J.360 Twist Handle Ratchet (3/8" DR) and the PROXXON 23 082 Rotary-ratchet (1/4" DR) for examples of ratchets with a driveshaft inside. Rotating their handle allows loosening and tightening of fasteners in zero-clearance / confined space situations. An even cooler ratchet is the DEEN DNR-60TWS: the grip end has a 1/4" female square insert so you can rotate the handle with a... power tool! Imagine the speed you can achieve with such a combo, while still having the whole ratchet head and handle stuck in an impossibly tight corner.

Interesting I just don't know what to think about it I don't see it for use in the auto world due to space confinement maybe possible but very limited but would still be great to add to the collection of gimmicky tools thanks for sharing

Needs to have a recessed hex bit head there instead for use with a impact driver or drill. Not to tighten obvs just to run the nut down.
