The one on the left is called Fixated and it’s from Nancy Wheeler. The top one is called Vigil and it’s from Quentin Smith. Quentin has some very bad perks but Vigil is ight. Nancy got some okay perks but nothing too crazy. Hope this helped if you haven’t figured it out already!

It’s a perk called Flashbang from the survivor Leon S Kennedy. But you can also acquire these type of items in the blood web during events.
