I'm a current neuroradiology fellow who graduated from RWJMS, which is how I stumbled upon this. Have got to say, seriously impressive, polished work on your case presentations. I can only imagine the hours that went into it.

It was suffering a thunderclap headache 3 weeks ago that raised suspicion for my GP. He recommended I go to the emergency department for a CT scan which showed i had CVST! I wish thunderclap headaches were more widely known about because it actually saved my life as it was likely my GP wouldn’t have responded so quickly if it was a general headache!

I'm an internal medicine resident and unfortunately, neurology is placed off to the side in our training, so these videos are so so helpful for teaching on neurology topics. Thanks Neurophile!

Doctor, I am a medical student. Today, a 69-year-old woman came to us and said that she suffers from slurred speech compared to the past, and that she has a curvature in the back, weight loss, and feels tired and she have hypotonia. And Mri was normal

Most people don't even get out of their house when hit by the Thunderclap, onset of pain to peak (10/10), less than 3 minutes, weakness, collapsed, nausea, instantly unconscious. Was on SSRIs. NASAH June 2015. Survived, thank God. 6 weeks in hospital (Salpetriere in Paris) ❤ who saved my life

I suffer from autoimmune meningoencephalitis (non-infectious) caused by meningoradiculitis ( nerve root meninges inflammation). My body now has trouble regulating normal CSF pressure, especially when changing positions. I suffer from thunder clap headaches often. I am in so much pain i am unable to move or ask for help. Arachnoiditis (meningitis) can cause this by damaging CSF pressure regulation. Spinal sanitation problems. The most common cause of spine and brain inflammation aka meningoencephalitis. Waste accumulation causes body to attack itself and when inside the, brain and spine, this can form lesions on various neurological systems as well as intracranial hyper and hypo tension symptoms. Spinal fluid flow obstruction is a likely contributer to thunderclap headache as well. Mechanical meningitis is one of three known causes of meningitis. Infection and chemical are the other two. Examples of mechanical meningitis is genetics, cysts i.e. tarlov, and physical damage like slip disc. Modern misconceptions ignore mechanical meningitis like i am diagnosed with. I found most doctors incorrectly defined meningitis as an infection not inflammation. My spinal and brain inflammation aka meningoencephalitis was ignored until it was self evident. By then it was too late for me. Infection is one of three known causes of meningitis. Not meningitis itself. Someone may have an infection in spine without inflammation ( meningitis). There is nothing in science that states meningitis is an infection but can be caused by infections, chemicals, and mechanical (i e. Genetics or physical damage). May I ask a question? What is the role of d-dimers? Should they be ordered for all patients with thunderclap headache as screening? And then if they are positive we will be more inclined to procede with a ct venography. Should we reserve them only for cases when we suspect aa venous thrombosis? There a lot of causes of eleveted d-dimers so there are a lot of false positive results. Nonetheless a number off the charts would point us in the direction of thrombosis and maybe a dissection too.. I am a new neurology resident just starting next week so i would really apreciate your thoughts on the issue. Thanks for all this hard work.

D.dimer is not a good screening test. It has a nice negative predictive value, but that's about it (we almost never use it for that purpose). Where d.dimer is truly useful is correlation with clot burden. If you have a patient with CVT and you start treating with anticoagulation, d.dimer begins to trend down giving you an objective measure that your anticoagulant is working.

Diagnosed in 2021 with vasoconstriction after aborted stroke. Just had second incident stroke/brain bleed. Terrified as told nothing we can do to prevent other than channel blocker and eliminate stress.
