Thank you for this !! As a new grad NICU nurse - i’ve only tried iv’s twice. Both times i got flashback but both got blown. I’m so glad I have seasoned nurses on my unit to help me out...I still have a long way to go

Thanks so much for watching Dee_V! Unfortunately, baby veins can blow a lot. All procedures are very humbling- just keep going! The only way you'll never be great at IVs etc is if you don't make any effort to improve! The mere fact that you're watching random videos on youtube to improve your craft shows that isn't you! I love that you say grab two catheters , whenever I am asked to come start and IV I say I need 2 don't open the second I just need to be able to see it!!

One thing I see a lot is the complete advancement of the needle. This will increase the likelihood of blowing the vein. The needle serves as the conduit for the catheter, it should not go farther than necessary.

Question 1)When putting scalp iv, do you shave the scalp? I'm from a developing country. Here they teach to shave the scalp first. Question 2) What about a very hairy baby?

The brilliant Arianna answered your questions (both excellent) 1) we typically try and not shave the scalp for a PIV. Instead of shaving, we try to part the hair with antiseptic (alcohol) where the vein is visible. After the IV is inserted and flushed, we use an adhesive around the site. Let it dry and secure you IV with whatever methods your institution inhibits. When removing the scalp PIV, remember to use adhesive remover so we don’t pull the babies hair out. 2) If baby is unusually hairy and you have no other IV options, shaving the hair may be necessary.

Doesn’t the 10 ml syringe have less pressure than a 3 ml syringe?

Hi Lee Ann! The physics involved in this question is kind of above my pre-med physics! The truth is the pressure generated is dependent on lots of aspects- including whether there is a vacuum in the syringe, the size of the barrel and how hard we pull back. Generally it takes more force to pull back on a larger syringe, so if it is just the same force (of pull-back) we are measuring then yes, the 10ml syringe will provide less pressure. But on pull back if the force were maximized with both, then that may be a different story! Not sure if this helps much!!

Is ultrasound typically not used for IV insertion in the NICU? Just curious.

Great question! More institutions are using quick U/S in the NICU for PICC line placements. We don't really use them for PIVs- often we'll use a transilluminator (red/ blue) which lights up area on wrist or foot etc making the veins much clearer to see. Thanks for subscribing!
