Wonderfully informative! I have been a NICU RN for 43 years & I agree with everything said! Especially about being nice! I have been yelled at too many times by co workers & doctors. It’s NEVER helpful.

I mean you’ve literally seen it all! Even medically- what do you feel have been the biggest changes you’ve seen? 43 years is such a wealth of information!!!

Omg such a gret vid. I'm only a year in to NICU nursing and people are way too loud in every room so Idk how to tell my senior nurses I think they're too loud. I might suggest that decibel tracker to a manager >.

Hi Dr Tala, Always enjoy your video. Like you, I believe in team work and I emphasis this to our students and new colleagues- I want to instill a culture of kindness and helpfulness on the unit, If I see a nurse with long finger nails when coming to clinical area, I will tell her outside the patient bay quietly. Our babies can weigh as little as 500 g so we need to be very vigilant. Infection control is vital. On a different note, can you do a video on how to interpret the monitor ECG waveforms? Obviously the monitor displays numbers of vital signs but how can we utilise the graph information to detact any issues for example, with potential electrolyte inbalance etc, this requires a skill in recognising the waveforms. We use 3 leads probes for babies attached to B850 carescape monitor that displays heart rate, resps; of course there are also BP, Sats leads attached as well. A video on all waveforms, what they mean will be useful. I teach parents not to fix on numbers as there are instances when the leads or probes are not even proply attached to babies. Thank you very much in advance.

I work as a travel nurse and am currently in NYC. I agree that conforming to the culture is vital; however, at this particular hospital I’ve had to educate countless residents who seem to write me off because I’m “just a nurse”. I refuse to do things that I know will give rise to potential harm. For example, pushing to bottle feed an infant on a PEEP of 5 or even 6. There is no IVH protocol for the most extremely premature, it’s frightening. I abide by the culture where I can and try to have a positive outlook on educating other staff on the many NICUs I’ve worked within and even learn a few things myself.

They allow you to po feed a baby on CPAP? We don’t where I’m from.

@@marylublankenship6398 they don’t just allow, they push for it. I definitely don’t do it as infants can silently aspirate among a host of other issues they’d be at risk for.

I’ve written a reply like three times and keep losing it! I’m so sorry you’re having this experience. Ranking a team based on letters after our names ended about thirty years ago. But this probably comes from a place of insecurity- like we often feel in medicine. I would say nurses taught me SO MUCH what I needed to know in the NICU in residency. Also- I know of some units that do feed on Cpap- there is rarely an absolute never in medicine- having said that- I would agree with you- it would scare me too! I hope one negative experience doesn’t put you off the NICU altogether! Good luck!!!

How do I as a grandparent let my concerns about noise be known. I feel almost, if not all, the nurses talk informal voi es over the incubator and when it's open. They talk loudly at times and LOL often just feet away. It seems they are just in the bad habit of not trying to be quiet. Our baby was next to the break room door...in ou in ou in out all day..WITH A LOUD CREAK! all the noise of rolling trash, laundry, instruments, etc...I feel it is hazardous noisy and it hurts my heart to witness it. My son and daughterinlaw are afraid to offend for fear of their child's care...they don't get that the noise is detrimental to his well being. I've left many times in silent tears and prayers. He was born at 24-25 weeks.. has a significant large PDA. He is now 31 weeks gestation with no improvement. He was able to go to the NAVA CPAP. Just seeking a bit of advice for coping here.

Hello- congratulations on your grandson! I am so sorry you're feeling this way, and I'm guessing the team would feel awful about it too. Often we're not even aware of how loud we're being. The entire NICU stay is based on relationships, so I would 100% say something to someone. Your baby's primary nurse, primary physician, the nurse manager- anyone you feel comfortable mentioning it to. Maybe ask if it's possible if he's the first one to get moved to another bed-space? At 31 weeks, a baby is still NINE weeks early. They have lots of growing and developing to do and it sounds like he's on track. I wish you peace and health XXX

No no I don’t appreciate that comment about older nurses being more rude you should see the younger nurses how they are with their patients 😢

Youre right Rania. That was a huge generalization and most places I work I haven’t really seen the “eat your young” stuff at all- at least not for 20 years or so. There’s definitely some generational stuff (that I guess has always happened- but I’m only aware of now). Slightly different values and different approaches. I’m so grateful for the younger generation for asking about sign out times and holidays and maternity leave- I never would have touched those in interviews. But then that can be seen as self-serving. I’d love to do a video on what we can all learn from each other so we don’t get burnt out (something my generation has failed at) but we provide the best patient care. Thank you for bringing this up!
