I'm Dutch and the name of the guy is Thierry Kok. Although a last name, the Dutch word 'kok' actually means someone who prepares food (a cook or a chef). Calgonit is a Dutch manufacturer of dishwasher tablets. They look blueish, that's probably the reason why the drink was named after this.

Here is a shooter I have made and I LOOOOOOOOOVE them You put it in a tapered shotglass. A generous pour of your faorite premium vodka that has been sleeping in the freezer. Pour a float of Galliano on top. Then top glass with a lemon wheel. Make a little mountain of bar sugar atop, then drizzle with 151 or Meyers dark rum. Carefully torch. When the flame no longer interests you, blow out. Then fold wheel with sugar inside and bite (like lime with tequila) and shoot. A GREAT RUSH sez the old hippie woman. I call it the "Flaming Brainfreeze"
