There was a remixed version of the same song which ultimately became a meme in the US Street Fighter Cartoon Episode entitled *The Medium is the Message* in which Bison unleashed his *"Trio of Terror"* on the Street Fighters, especially Guile, and then uttered his famous *YES!!! YES!!!!!!*

I recall a Roadrunner and Coyote game in C64 or Amiga that used Toccata and Fugue in one section. As my 10 year old brain remembered it at least, it was an excellent version.

Another one that uses the full song is Fantasia on the Genesis. It's used in the opening and the final stage. As for others with Snippets, there' Bloody Tears (Castlevania series), Behind Pentagulia (Lunar 2: Eternal Blue, also Brilliance from the first Lunar definately carries the same vibe at least), Dancing Mad Part 3 (Final Fantasy VI), Flugel (eXceed 2nd: Vampire Rex). Also Ultimate Koopa from Super Mario 64 was definitely inspired by it.

I just remembered that there's also a bit of Tocatta and Fugue in "Ode to Ancestors; 8th Movement" from Mother 3. In fact, that entire track (as well as the track Family Matters: 2nd Movement) are both made up of bits and pieces of Iconic. Hell both of them open with the intro to Beethoven's 5th symphony.

I just thought of a few more after seeing that you hearted my comment (thanks by the way). A more subtle use of this is in the track "True Power of the Occult" which was used for a brief Yandere Simulator mini-game that parodies Undertale. The 4 note base line is the Tocatta portion but slowed down. Considering the general vibe of the track, it's likely that this was intentional. There's also the track "A Tocatta into Blood Soaked Darkness" from Castlevania: Curse of Darkness, which is pretty obvious given the title of the track. The title theme to Lightning Warrior Raidy 3 also uses the Tocatta portion. Although I should warn that the game is pornographic in case you decide to look it up.

Came here for Captain Cosmic, was not disappointed. Although I got surprised that Genesis' Fantasia wasn't included... I mean, if you think about that game, this music instantaneously comes into mind. Also, some parts of Tocatta appears on some Castlevania's music.

It wasn't a game, but one of my favorite uses was on Oh-No's "The Disrupt" from '04, the track "Move". That album has a dope video game sample, but I always forget if it's Ninja Gaiden or Castlevania that he sampled.

My favorite is the Gyruss C64 version. It's so well made. I also liked the version in Killerwatt (for C64 as well). It's a little bit rougher, but it works well with the pace of the game and makes it feel exciting!

Bach's works seem to be very popular with video games lol his little fugue in g minor is very prominant in megaman legends' final boss, and it is level 5 I think in the game Cathrine.

Atari ST: Thunderstrike intro Dark Castle (not sure if you got that version of the song - it's nearly identical to the Amiga one, but only plays during the main menu, not during gameplay). Also, fun fact: the music in Gyruss is actually a cover of Toccata as rendered by the English/Australian, instrumental rock group Sky, as there is a section in both versions not heard in the one by Bach. Also, I think maybe Bloody Tears from Castlevania 2 might have been inspired by the running 16th note section.
