Tony saying he doesn’t care to flatter her is basically exposing what he really wants. Dr melfi is the shiny toy he can’t have and the mystery of it/her is only increasing his urges. He’s also used to getting whatever he wants one way or another, so being rejected is new territory for Tony which is why he asks her to help him understand and seems so child-like in the situation.

I think there were plenty of times Melfi was genuinely attracted to Tony. Even right here. The biggest difference is that unlike Tony, Melfi is in control of her emotions and urges and is mature and professional. She doesn’t go around indulging in anyone she’s attracted to or intrigued by like Tony.

I don't think this was so much about the rejection here but the reason the Melfi rejected him that made Tony react like that. Tony knew that Melfi has a distain for his mob lifestyle and as a therapist you are supposed to refrain from passing judgement. Tony was actually very clever here in teasing that out of her. She was trying to be professional but he wouldn't let up until eventually she showed her hand making her a hypocrite in Tony's eyes. He was very manipulative.
