Thank you so much! :'( You don't know how long I've been wanting to learn this pat down especially since I'll be taking the boards soon. In almost all the other videos I've watched, they spoke in really highfalutin medical jargon that made it difficult to understand. The books didn't help much because they weren't able to deliver it the same way as you did. Yours was just right. I needed this.

Best video on this. So I have a question, if a patient has pulmonary edema would you give the patient a hypotonic solution ?

IV diuretics would be the first line... and other tx and fluid choices can vary based on if it's cardiogenic or non.

So what about a patient with severe burns? you would give iso or hyper? Or a patient with heart failure?

For severe burns you would administer an Isotonic solution - in severe burns the patient is Hypovolemic (this means the patient has a low blood volume and it is due to the water loss he sustained from the burning/flames). In this case you need to increase the blood volume so you would administer an Isotonic solution.

You usually give them Lactated Ringers (An isotonic solution). Burn patients usually lose fluids as well as electrolytes (usually sodium and potassium). Lactated ringers is the chosen fluid because it contains not only water, but also Sodium and potassium which can essentially replenish whatever the patient has lost due to fluid shifting (with burn patients, fluids tend to escape from one body fluid compartment to another, hence, the electrolytes often escape alongside the fluid. EX: blistering or 2nd degree burns). Potassium and sodium are essential for life; Remember that potassium helps with heart relaxation or depolarization while sodium helps retain fluids within our body.

So…Thanks for the awesome video but if you could please help me verify this: Dextrose 50% is a hypertonic solution aiming to increase serum glucose, but administering it will cause the cells to shrink. the only contraindication for this is intracranial hemorrhaging, and is the reason behind this because we do not want to shrink the cell because the brain will be in need of the blood cells? Thanks!

Correct- Dextrose 50% should NOT be administered. This will decrease plasma osmolality and so increase the water content of brain tissue (the blood brain barrier acting as a semipermeable membrane).

tootRN, LLC. Interesting, I've heard intracranial hemorrhaging has been deemed, by some, a contraindication for administering D50 due to the hypertonic nature. As the thought of necrosis will occur. I'm learning that (especially in the case of a hypoglycemic patient) you push the D50, because by the time the D50 reaches the brain cells it will be diluted enough where contraindications will not occur. And the brain cells will receive the much needed glucose.

Hello, I am confused that why since Hypertonic solution can drag fluid from brain cell during brain edema and cause hypervolemia in blood vessel, but if we administer hypertonic IV to patients, we have to watch out for pulmonary edema. It looks like one side, it can decrease edema and other side can cause the edema.

+Emily Polite . Burn victims lose a lot of blood. According to what she said you give fluids to someone who's lost a lot of fluid. Since LR is a isotonic solution, LR would be a good choice along with other isotonic solutions.

I'm not sure about sth. You said hyponatremic patients get hypersonic solutions. We know that hypersonic draw the water from cells to plasma. And making plasma more watery. This will cause LOWER CONCENTRATION of Na. But then you said Na follows the water comes from cells and that's why Na levels at plasma will increase. Here is the thing that I don't understand : I thought water following Na. Not as "Na following water" the reason I thought this is water pass every membrane by osmosis but Na needs gates. So the logical statement should be : The water follows Na. The one which can pass every membrane follows the one which can not pass every membrane. Help me pls. I am so f.ckin confused....

Oh shoot! I did understand!! The given hypertonic solution is %3-5 NaCl. So we are giving more Na pluuuuus causing water efflux from cell and that's how we are balancing the %3-5 Na. Because the given solution has soooo much Na inside that's why we need water from cells. To make it 0.9 again! Oh!! * I love you ppl! Thx for vid. BUT I STILL THINK WATER FOLLOWS Na :D
