Very nice. I always teach PFMEA using the standard assumption "assume all incoming material is good." However, your presentation and my experience with quality issues clear illustrates why this is wrong.

very clear explanation. Thank You. I have questions from my staff on Severity point . we found a lot of samples from others industry especially from internet or automotive that severity point is not change even action plan already taken. My understanding severity point should be change base on new action point of view. Any idea?

In regards to mistake #6, what ranking guide are you referring to? As per the AIAG FMEA Manual 4th edition, a ranking of 3 is "Appearance or audible noise, vehicle operable, does not conform and noticed by many customers (50%)."
