Topical Finasteride: Same Results, No Side Effects?

In recent years, interest in topical finasteride has exploded. For people with androgenic alopecia, the hope is that topical finasteride comes with the same hair-promoting effects as oral finasteride, but at a lower risk of side effects - since the medication is applied topically instead of taken orally and thus "remains" in the scalp.
Unfortunately, this assumption isn't true. Even worse, popular articles and videos about topical finasteride seem to focus mostly on its "percent dilution" - 2.5%, 0.3%, 0.15%, etc. But these numbers are meaningless by themselves, since they're just part of an equation to calculate total daily finasteride exposure, which we will need if we want to effectively estimate systemic leakage rates of the drug.
Without understanding these formulas, exposure rates, and estimations for systemic leakage, many people using topical finasteride end up over-applying the topical and causing reductions to dihydrotestosterone (DHT) everywhere throughout the body - often at the same level as oral finasteride. This defeats the purpose of paying a premium for the topical! And this can occur with application margins as small as 0.5-1 mL.
Fortunately, there may be a way to maximize the hair growth-promoting effects of topical finasteride while minimizing the drug's risk of side effects. There's also a formulation of finasteride that appears to have minimal (if any) effects on serum hormones like DHT.

I'm confused. You said the 2014 study the topical finasteride 0.25% x 1mL reduced serum dht by 25%. Then in the overall chart it says that 0.25% x 1mL reduces serum dht by 70%. Maybe I am missing something or making a mistake here? Can you clarify?

Hi Rob. Yet another great video. Quick couple of questions based on everything covered in this video. Many years ago I was taking Finasteride in pill form and I did suffer a little with side effects. I was only on it for about a year before deciding what o come off of it. I do have fairly advanced hairloss and have tried many options. However, I am on the hims regime now and have been for about 4 months. This is their combined topical Finasteride and Minoxidil solution, with the Finasteride dose at 0.5ml. I’m in the UK and they work on the principle of topically administrating this 0.5ml dose twice a day. However, I have only been applying a single dose of 0.5ml once a day in the morning. So although I’m not ideally using it to the company’s directive, it has had an effect thus far. I am also derma rolling once a week, so maybe that’s helping too. So based on your video info here, I feel that I may stick to the single daily dose rather than squeeze in a 2nd each day. It helps with cost of course, but with the percentages you mention, the single dose may well be the better balance for me, especially as the pill all those years ago did obviously have an effect on my overall DHT. However, I’ve been using the solution for around 9 months now and I haven’t really had any positive results, which is a shame. I suppose I must be one of the 10% that it doesn’t work for. @@sanktxando First, I am not a doctor, and any information given is not intended to treat or cure any medical condition. I crush a quarter piece of finasteride into a bottle of liquid minoxidil. If you use 2 drop fulls of minoxidil a day, then multiply the percentage to be used (0.02%) by 30. In my case I use (4 )1 ML drop fulls for total scalp coverage. If only 2 drop fulls then crush half a fin' tab into the minoxidil.

Interesting. I just started taking Saw Palmetto because I heard that there were studies showing that it decreased DHT and improved hair without side effects. I also started using a shampoo with Rosemary that supposedly lowers DHT. I've started using Minoxidil on top of that but only about half a milliliter (maybe less) twice a day since I'm only experiencing recession of the temples. So far no side effects. Hopefully this helps prevent further hairloss

Fantastic vid! Have been considering going topical but you drew attention to factors I haven't considered. I take 0.31mg of oral twice a week as I believe myself to have combination hair loss (genetically lucky towards AA but with a very sensitive scalp that causes excess shedding). I find it has really worked with a daily loss down from 150+ to 40 approx. Question is...does this still cause sufficient issues systemically iyo? Hi Rob - Excellent video. You really got me thinking about all of this and I am now encouraged to try it. As I prepare my request to the doctor for an Rx, I have a couple of things I want to make sure I have correct. There are two things to consider: 1. the actual concentration of the topical. 2. The total daily exposure to the drug. So you're saying a concentration of .005% (w/w) will provide enough potency to help with hair loss and a total daily exposure not to exceed .2 mg. Is this correct ? I cannot find the study that shows the effectiveness and DHT effects for the .005% concentration. Can you please provide a link ? Careful with the dht blood tests. You might see a difference of 30% just taking the test on different days. You would need to take several tests to get an average of your typical dht before you can know what effect finasteride had.

Within the first 7 days of having topical Finasteride and Minoxidil via Hims, I definitely overdosed. It's so difficult to put the recommended dosage of product onto my head because I have a lot of hair. Using the provided sprayer is way to much product so I got myself a 1ml dropper. I was putting 1 - 1.5ml on my head per day. On day 5 I got a screaming headache that wouldn't go away and I rarely get a headache. I need to get a smaller dropper and I'm going to try putting .25ml on my head, maybe every other day. I'm planning on waiting for this headache to go away before I resume usage at a dramatically reduced volume & frequency. Great video!!

This little topical finasteride might explain the next. Rob, please I need your opinion on this. What do you think about the thing that a little of dht is needed to kickstart hair follicles? The thing is that so much people who are perhaps a norwood 2 in their 30's will only stop hair loss while taking finasteride and guys with norwood 3 to 4 in their 20's hyperrespond to taking finasteride. The thing is that the androgen receptor is probably binded too often and way too fast with dht and the other receptors(vit a, d,...) cannot keep up. When those are empty, i think cell death is displayed. Otherwise when they're filled and then dht is binded cell growth is displayed. It bassicly means the androgen receptor pushes all the right ingredients in the follicle, but when there is nothing to push in, the follicle will miniturize. Oral minoxidil is known for it's very potent hair growth potential. It brings the right ingredients to the hair follicles. And sometimes hypertrychosis happens as a side effect of that. But you know what's funny about hypertrychosis, it's usually right under the hairline. It's probably because there is some dht there (I believe this because of the constant inflammation through muscle pulling) to kickstart the sleeping follicles. This also might explain why topical minoxidil only delays hair loss, it will miniturize at a slower rate because the right ingredients will get binded more often before DHT can push it inside the follicle. Also remember, everyone has dht in their scalps, not only people with AGA

I'm always open to learning alternative perspectives on the pathological step-processes of hair follicle miniaturization. I can say that it's incredibly complicated. It's very interesting that DHT is associated with hair growth in the chest and face, but that the opposite tends to be true in scalp skin. Why the opposing roles of the same hormone? Why does DHT in scalp tissue dysregulate wnt/beta-catenin pathways and induce TGFB-1 in hair follicle sites, but not behave the same way when bound to cell sites in secondary hairs acquired during puberty? Why does estrogen seem to be the key hormone that triggers hair growth in pubic regions (i.e., armpits and groin), but not in the chest and face despite the hairs having similar characteristics? There's a lot we don't know, and nobody yet has crystal clear answers. I would need to see the sources that support your comments above before providing my insights, and that sort of exchange is less suited for KZbin comments and more so a conversation.

This is a thoughtful video, but this study was over 25 years ago with only 52 patients in which only 36 completed it and self-scored themselves with some improvement while the rest dropped out because of lack of any improvement in the status of the scalp hair. There were no stats given on serum testosterone measurements at baseline and throughout, and only a statement that no one reported any side effects. It would be nice to have something more concrete in modern times to support this recommendation with support that it works. Some comments below, and even a search on this study in other very popular hair loss forums, say this protocol either still brought on side effects for them or didn’t do anything at all. Any attempts to get updates on, “is this working with no side effects?”, usually go unanswered in those forums and those threads are short-lived. Are there any hard data or concrete experience testimonials that this protocol works without the side effects of the higher dosage? I would like to try this after an unpleasant run with full-dose oral Finasteride but I just don't see any other compelling proof anywhere that this is worth the trouble?

Personally I consider the easiest way to obtain low-dose topical fin (cheaply) is by crushing 5mg of Finasteride pill(s) into a powder, and adding it to 60ml of Minoxidil, which will bring the dose very nearly to the one recommended in the video (it would actually be slightly weaker, at around 0.0833% per 1ml, which I consider acceptable since I combine the mixture with a small amount of tretinoin cream to increase absorption.

@@chriscorbartUnfortunately, I was not a very strong responder to finasteride, and ultimately decided to cease using it and rock the shaved head look instead. When I was taking it, I would use it once a day, usually at night before bed. I generally used a little bit more than 1ml, sometimes 2ml to ensure full coverage, since I figured the dose was low enough not to matter much. I never experienced any side effects in the year and a half I used it, and I did get *some* regrowth, just not enough to really justify the effort, at least in my case. I ended up using Minoxidil foam on my face instead to promote beard growth, along with a small amount of tretinoin and microneedling, and that had a *significant* effect.

So if I crush 10, 1mg finasteride tablets into my 90ml solution of 50/50 PG and Alcohol, then thats 10m/90 which is .111mg per ml? So what's the percentage of that? I usually do a dose as 1 ml so am I doing it correctly? It seems like it. I am now adjusting my topical formulation as I didn't realise how much of the topical finasteride was going systemic and the extent of DHT reduction with regards to systemic finasteride. I was applying 1.5ml of 0.15% topical finasteride / day which is equates to 2.25mg finasteride and assuming 25% (worse end of the scale) of this goes systemic then this equates to 0.56mg. This would as good as cause maximum serum DHT reduction. I recently changed from 0.5mg /day oral finasteride to topical finasteride to avoid this and topical is exponentially more hassle than oral, let alone the additional cost!! @ Rob - regarding DHT testing what would you recommend in terms of a finasteride (that has been used for years) cessation period so that the body can return to baseline DHT levels? Great to hear! I think that’s a good plan. I would shoot for a one month cessation period. That won’t be enough time to start losing hair again (in most cases), but it should be more than enough time to clear finasteride from the serum and get a baseline DHT reading. You may also want to just test your DHT levels prior to stopping, to get an idea of your own systematic absorption rate. It varies wildly for lots of people.

Excellent video. The thing no one talks about is how much to put on your scalp when you have a lot of hair. It’s a lot more difficult to penetrate your scalp. What do you think?? I got a hair transplant back in January with great results. My hair transplant doctor told me if I want to go with the pill form that 1mg oral finasteride is complete over kill. He recommended .25 to at the most .5 mg.

Fantastic Video, I started using my .25 topical twice a day mixed with 5% minoxidil, and have zero side effects, but I assume due to my mood that my testosterone has risen a bit. So I am sure I am absorbing a bit of it. Nonetheless, I overall am doing well on it, so I might talk to my doc about 1mg oral Finasteride and switch to 5% minoxidil topical only.

I've heard of people putting 1 ml of Hims 0.3% into a 60ml bottle of generic minoxidil and then using 1ml of the final solution per day. I haven't tried it yet but am interested. Please correct me if I am wrong.

I was using 1 mg everyday for many years. I switched last year to 0.1% finasteride mixed with 5% minoxidil twice daily, and 1 mg orally 3 times a week,and got waaayyyy less side effects with a better result on hairline.

@@BiZii1024 We've got guides on oral and topical finasteride, as well as oral dutasteride. The topical / mesotherapy guide on dutasteride should be out in 1-2 weeks. Interpreting the research here is a bit complicated, and we have members who've gotten contradictory data on systemic leakage from topical dutasteride versus the available clinical studies. I wouldn't join the membership just for that guide, but there are many other resources there if you're looking for alternative strategies for DHT reduction!

I've never seen any big company claim zero sides, if topical. But it stands to reason, as with any drug, not taking oral and going through liver, is a good thing and reduces possibility of sides and more site specific.

Hey rob, your email series and the articles on the site have eally been very helpful, thanks for all of it. Actually, I m a seventeen year old, and have aggresive mpb, my hairline has receded and and I have diffuse thinning all over the scalp. I have been on topical min5% and fin 0.1 % 1 ml twice for almost 7 months now. I don't think have I have been responding to minoxidil well as my hairfall continued till somewhat 6 months with breaks and it's only recently that it has kind of slowed down, I don't see much hairs falling now. But over the time my density has got a lot lower, and although the hairloss slowed, no sign of regrowth (may be a few hairs on hairline) but crown has got worse I think. (Ps I started using onion juice twice a week, and I suspect hairloss has slowed because of it) 1. So What do you think I should be doing, 1. Add dermarolling and wait for topical fin(and minoxidil) to show it's effects? ( it might be taking time, as the hairfall has stopped recently? 2. Or go for fin tablet (and would you recommend fin tablet for me(17, will be 18 in two months) although I think I have grown enough.) And if I do start the tablets, will I go through a shedding phase again? 3. And since my aga is so aggresive, do you have any other advices/recommendations?. 4. And just confirm umm does finasteride have any effects on conginitive func etc?

Buy a Dr Pen with the 16 needle tips and use once a week at 1.5mm. It moves almost everyone to a high responder to minoxidil. There are a lot of videos on it, but basically 5 passes over each area in different directions or until the scalps looks pink and irritated with some pin point blood spots is enough. It should only take a day or two for the redness to go away. Only need to be done once a week. The tips are one time use.

I decided to start fin 1mg 3 times a week orally. After the first pill, i had all the side effects (ED, chest pain , mood swings, itchy balls). I stopped it immediately, amd After 10 days of stopping the drug every thing went back to normal. Now i decided to restart it topically, i am using 0.2 mg per day (0.02% , 1ml per day) and i have zero sides. I hope my hair responds well to this topical dose.

That's great to hear! Congrats on taking the time to troubleshoot this and finding a dose that works with your body. For what it's worth, the study on 0.005% topical finasteride at 2 mL daily showed positive hair outcomes over 16 months. That suggests your dose (provided you're using 0.5-2 mL daily) should also help.

My dht ( I’m on trt ) was 95-102 on my past blood work. After adding hims 0.3 topical it dropped my dht to to 32. I was only using 3 instead of 4 sprays. Haven’t noticed any sides but seems to be pretty aggressive so I will be finding a lower dose
