is there a way to counter an agent card like an assassination. or can it fail somehow?

Hi.. Indeed you roll a red dice. On a skull (1 in 3) the attempt is succesful, but on a wound result (1 in 2) your agent is killed in the attempt. An S is a fail but your agent escapes

Yes, a normal murder action rolls a red dice and can miss. This was a special ability that picks and random army and skips the roll instead.
