Here's my theory on the Great Maw's origin,the warpstone that was forced down onto the ogres mutated their many dead(hopefully)bodies and then amalgamated them into a huge mass that formed into the crater and with the combined hunger of all those dead(hopefully)ogres and the combined belief of the ogres that are still around created the Great Maw.

Since Warhammer Fantasy and 40K are NOT supposed to be in a canonically connected Universe, its still worth a thought: What IF the great maw is just a Tyranid stuck in there, constantly being fed by Ogres and feeling content with it. Maybe it got seperated from the hive mind in some way and does not have a reason to run rampage whatsoever. If you look at the design in TWW3, the GM does resemble a Hiveship Maw in some way. I cannot unsee it

Chaos worshipping ogers and maneaters were models long before ogres got their own line of miniatures. Giving theie prevalence in the warhammer world it’s reasonable to assume chaos is the 2nd largest religion among ogres, un aligned is probably 3rd largest demographic. We don’t know how many ogres live in cathay or nippon but they’re probably negligible.

The number of ogres in Cathay may not be that small. They people of Cathay have long since employed Ogre mercenaries and even recruited some into their own forces. Tempting them with weapons and gunpowder. At least as I remember it.

Not officially as far as I know, but CA is limited on which official factions can be released in the next game and it only makes sense for the Ogre Kingdoms to be one of the starting four factions if they do not decide to make a completely Chaos themed game.
