There are 3 characters in Touhou that have confirmed descendants -Junko (rip her son) -Suwako (Sanae) -Sumireko (Renko) the only question is who they all did it with

Renko is not a confirmed descendant of Sumireko. They are implied related somehow, but we do not know how. Junko was married to the emperor who was then usurped by Hou'yi, so we know it's one of those two, likely the former. Suwako is a goddess and if births follow traditional shinto god births then she didn't have to do it with anyone and Sanae could be a descendant of feces like Keiki for all we know. Seiga also had a family that likely included children. Miko also had children. And if you really want to stretch it, Marisa's Father is a known character, so you could say he has a confirmed descendant. Same for Kosuzu's grandfather.
