Trabeculectomy, also called Filtration Surgery, is a surgical procedure performed for treatment of glaucoma. The treatment involves removing part of the trabecular meshwork and creating a new escape route for the aqueous humor. When successful, it allows the aqueous fluid to drain from the eye into an area underneath the conjunctiva where it is subsequently absorbed by the body's circulatory system or filtered into tears.
In this procedure:
- A conjunctival pocket is created and maybe treated with Mitomycin or other antimetabolites for a few minutes. These drugs are used to prevent scarring of the operation site. Scarring, if occurs, may clog the new drainage canal, and is therefore the major reason the procedure may fail.
- A half thickness flap is then made in the sclera and is dissected all the way to the clear cornea.
- A block of scleral tissue including part of the trabecular meshwork and Schlemm's canal is then removed to make a hole into the anterior chamber of the eye.
- As the iris may plug up this hole from the inside, a piece of the iris maybe removed at this time. This is called iridectomy.
- The scleral flap is then sutured loosely back in place. These sutures can be released gradually during a couple of weeks after surgery. This allows adjustment of the aqueous flow in order to achieve target pressure and to avoid the complication of having a too low intraocular pressure.
- The conjunctiva is sewn back in place to cover the area.
After surgery, aqueous humor drains into a filtering area called a "bleb" under the conjunctiva. Since the surgery is usually performed near the top of the eye, the bleb can easily be concealed behind the upper eyelid.

My son had this surgery @ 7 months old, that was in 2010, he’s done amazing since, no issues with vision, hasn’t had to repeat the procedure yet either. Amazing stuff.

Hi I was operated at 1997 in both eyes .no issue yet.do u use any drops for your son after surgery.some doctors advised me to use drops but I am not interested on life long tratement .

What 'll happen to the bleb ? Is it gonna stay there? If yes ..how does it justify the purpose then coz it'll keep on accumulating the fluid .?

I had this operation in both eyes, and at the renowned Cleveland Clinic. Unfortunately, they both failed after only one year, and the result was a spike in my eye pressure that was worse than ever.

@@williamgallos2570 I later had Baerveldt implants (glaucoma drainage tubes) in each eye. I still have to take methazolamide daily to control pressure.

We often use our eyes to equalize intraocular pressure, but after injuries and surgery, you can't do that... You just can't cure glaucoma with drops, you need to change your lifestyle... The pressure in the eye is increased due to the spongy Trauber network clogged with fibrous blood, which removes 75% fluid into the Helmet channel. This often happens after getting blood due to eye injuries, or eye surgery, sugar, malnutrition and... etc.. There is more fluid entering the eyes than leaving.... Which really helps! It is necessary to improve fluid drainage, but how?: 1 Improve rheology (fluidity...) diet (for example, diabetics), cans to improve lymph flow, leeches are the best... and so on 2 Improve blood vessels (contrast shower!!!, vitamins C and much more ...) 3 Mechanically help to remove fluid from the eyes (massage around the eyes to drain lymph ..., alternately squeezing- squeezing out excess fluid if you can not rub your eyes due to surgery or injury, etc.) ... Everything is correct. But not all of them... I'll add it... In order for the blood to be "liquid", proper nutrition is necessary!!!, without excess sweet, flour, potatoes, etc. A good outflow is helped by proper MASSAGE of the eyes and adjacent areas and neck. But it also helps a lot to alternately close your eyes!!! An eye to squeeze out!!! excess fluid from the eyes into the lymph via the trabecular network (spongy tissue is clogged with fibrous blood after surgery or injury), into the Helmet canal. Massage of lymphatic vessels draining fluid near the eyes and ears, as well as from the entire head helps to prevent and treat GLAUCOMA!!! A daily contrast shower helps, or at least applying hot wet or cold wipes to the eyes to train the elasticity of the vessels around the eyes, and the entire head. All this prevents GLAUCOMA, which is almost impossible to cure in any other way. Only if you put leeches to thin the blood. Regular cupping and cupping (hijams) also help to improve the rheology of blood and lymphatic fluid.

his pressure went down tremendously with the drops he's taken (combigan and lumigan) so his doctor has postpone surgery. he's been goin to the doctor every 3 months to keep watch on his pressures hopefully he doesnt need it ....

I have had this done in my left eye and I had to have it redone after a four year period, but my pressure at time is low and causes blurry vision, but when the pressure is above 7 my vision is okay and the vision is great. Like I said it has a four to five year window.

+Ali Rezaie Visit an eye hospital ASAP its actually the doctors job to give u advice. If your baby is having congenital glaucoma treat it immediately.

My Grandmother had Glaucoma and we took her all the way to India to do surgery for the both eyes last year, but but after spending one month in india we are told that she is late and now both the eyes are blind. Can someone come to her aid we are from The Gambia West Africa. Thank you

Can you be put to sleep for these surgeries? The reason I ask Is, I have high pressure in my eyes right now-21. The doctor Is monitoring the Left Eye In particular. She says IF I got Narrow Angel Glaucoma, The specialist does a STINT surgery where they Implant a stint to prevent closure. This whole thing would scare me to death. Any Insights are very welcome & most appreciated. TYIA, Amy

In principle, yes, but it's up to your doctor to decide. Besides, that comes with additional risks that may not be worth it. Sedation is probably enough to calm your worries.

Had this procedure 4 days ago. No pain and very little discomfort, just a little soreness. Certainly nothing to fear from the surgery for any reason.
