ALWAYS ALWAYS AWLAYS pt in at LEAST semi-Fowler (CHEST at 30deg), or if in doubt, FOWLER (CHEST at at 45 deg). NEVER flat, that is JUST crazy shit! CHEST at angle, NOT HOB! Where do you get your Hg settings from? What book, what manual?

bad suctioning ... sorry u sude know what size is the trachea in order to not go to deep if the patient has caughing reflex then with the first cough u have to slowley remove the catheter out and the position of the pation is wrong ,if he start to vomit it's high risck of aspiration and not only .

No personal protective equipments ( mask, uniform) no bacterial filter on the Ambu-bag reservoir. This nurse wears rings and has long gel nails, this is not allowed in a hospital and she did not provide hand hygiene before suctioning ...., and position of the patient is not suitable...

Thank you for your feedback. This is performed at a school. There is no mask so the students could hear what was being said. In the beginning of the video it was said that the hands were washed and the patient was put in semi-Fowler’s position. The bed was not working. The instructor has been a critical care RN for 27 years. There is no policy against jewelry in the US. There is a policy regarding nails. Those are not artificial nails. Your comments are appreciated.

It really depends. I have had patients who sprinted while suctioning and have been threatened to be stabbed in the neck with a fork if I pull it out before I am done suctioning. Yeah, to be safe, 10 seconds but it is not always true. I have had patients with tons of secretions and 10 seconds wouldn't do much and they would be greatly upset if I didn't keep going and get the secretions out of their trach.

This is the worst one I've seen. Bumping them twice would push down anything your trying to suction out. And laying flat, that's just such a big no no

MyNana420 thank you for your comments. At the beginning I do mention to raise the HOB. The bed was not working. As a nurse of 27 years and 15 of those in critical care I must say this is one of the weirdest ones I have done! As the students say in simulation it is difficult to “act”.

This is absolutely ridiculous. You do not just 'meet resistance' there is specific measurement so you don't cause discomfort and trauma. There is zero point going further than that. You should not be doing this video. I have seen nurses do this because they had 1 workshop, no trachy nurse ever would. You are in a hospital setting and i'm pretty sure with his next LnB his ENT specialist would have an issue because it shows up on their xrays if this kind of thing happens. You are wrong and should NEVER teach this because people with temporary trachys could end up with permanent problems and others with trachys will have pain and discomfort which is not necessary.
