That’s going by G1 cartoon timeline back when the franchise was still new. The 13 in this vid and their story were created for other continuities and, moving forward, are pretty much now a constant occuring legend for all future Transformers stories.

Depends on the series. In most he wasn't, but starting in Prime the made it so he was. It was then changed again in IDW's G1 stories with Optimus being the first Prime since the 13 and posing the question of if he truly was the 13th or not in the form of him being worshiped by those who thought he was.
