Wonderful presentation as always! I get the notifications for these videos and eagerly go to them. I actually was questioning myself as to why I do this when I am not a medical person and a large amount is way beyond my comprehension. I think it is because your passion for what you do is just so inspiring to see. I really think that obvious passion is not all that common! And I always do learn things! On the gangrenous gall bladder - I remember my Uncle ‘Eric’ having a gangrenous gall bladder and how serious it was at the time ( late 1960s I think).

Thanks so much Nora! Yes, today a gangrenous gallbladder is life threatening if not managed appropriately…in 1960 it would have been near fatal. So happy you enjoy these videos, I certainly enjoy putting them together! If there is ever a topic you’d like to hear more about I’d love to put together a video on it.

It should be noted that often the pain from cholecystitis is not in the upper right quadrant but in the epigastrium like mine.

In cases where there is a stone in the CBD, I guess you would not think about chemical dissolution using MTBE or Ursodiol. As A surgeon, I guess you consider a cholystectomy as the "gold standard".

Not all NSAIDs work. Ibuprofen did nothing what so ever. The number 1 thing to do when a patient presents like this is to administer a pain reliever ASAP. The pain from this condition is off the charts and constant for hours. Let me emphasize constant. This kind of pain could kill an elderly patient. Why make a patient suffer needlessly. Stop the pain and go from there.
