DJT’s candidacy…it’s deeply concerning… There are lots of mixed messages by media and a dangerous ignorance of American society in particular, for recognizing neurologic disease and psychopathology, let alone personality disorders…of which he has all three. In addition to being a hospital based internist, with a comprehensive approach to acute, subacute, and chronic medical illness and pathology, I’ve had a semi-academic interest in neurological disorders and psychopathology for decades now but even most of my colleagues struggle to recognize and deal with behavioral pathology on a day to day basis since they usually don’t pay attention or study beyond our specialty specific academic requirements. DJT would not be fit for making medical or financial decisions, even though he is volitional. In my opinion, he has signs of vacillating between mild to moderate signs of frontotemporal dementia - paraphonemic paraphasias, apraxia (as evidenced by fatigue in speech and maybe some logopenic features), increasingly simplistic modes of comprehension and judgement, emotional dysregulation (with supranuclear systemic involvement), obvious declarations of apathy, disinhibition w/vulgarities, declining muscle control and balance, some postural muscle dystonia and core rigidity (has intermittent thoracolumbar postural dystonic movements and limited ability to turn his neck and upper body), along with some features of psychosis (loose associations, flight of ideas, persecutorial perseverance) accompanied by delusions…all this with malignant narcissistic personality disorder with dark triad personality features. Just the extent of confabulation alone is cause of grave concern. In addition, from a medical perspective, he has probable OSA, circadian disorder, likely MAFLD (metabolic dysfunction associated fatty liver disease), and I’m sure a few other prominent medical illnesses based on observational assessment of available videos and pictures. Talk about being unfit… And there is a massive sociopolitical movement organized behind these raw and unfiltered features, that promise that lack of civility and logic, with an open display of anger and hostility that are not only acceptable, but are desirable as being uniquely American. The culture war on expertise, knowledge, civility, and social organization has more or less become the Republican platform combined with the elements of Christian superiority platformed in righteous domination of others as an unalienable right worth fighting for. Truly disturbing. I worry for this country…I really do.
