+Ariful Islam From a strength standpoint, a slip knot or even a series of them is weaker. one string wrapped around the other stationary strand a bunch of times puts much stress on the straight string increasing the chance of failure. hence square knot, AKA surgeon's knot in medical field.

Why do we alternate between the index finger and the thumb at all, couldn't we just continually use the index finger?

Hello David... If you use only index finger, youl will end up wit several simple knots on top of each other. They do not hold well, will slip and twist easy. The Square knot is strong because the second knot is in reverse. Both knots are tying each other together. The symmetry of the two strings coming out of the loop, is the build you are looking for. Still, I find that there are much easier techniques for doing square knots than this- by simply doing a right-over-left, and the a left-over-right. I find that this sort of technique makes it much tougher than it really needs to be to get the same sort of knot. Mind you in actual practice, you would be using needle holders to do this more than likely, in view of sharps safety. I get that we're trying to use a "one-handed" technique, but I don't see why this is necessary or functional- it just seems to make things harder to do in general.

You have to alternate to get enough friction and stability. One use of NOT alternating knots could be the slip knot. You will use two throws of the same direction to get the ability to push the knot into the depth of wound. but you still have to fix it with alternating knots afterwards for a permanent strenght.
