A solitary, large oval hyperechoic mass in the right lobe of the thyroid with marked internal and rim vascularity on ultrasound is most likely a follicular adenoma. This benign tumor often appears well-defined with smooth or lobulated margins. On color Doppler, its internal vascularity suggests a highly vascular nature, while rim vascularity supports the diagnosis of an encapsulated lesion. The hyperechoic nature, resembling the appearance of testicular tissue on ultrasound, further suggests a macrofollicular pattern. Though generally benign, follicular carcinoma must be considered in the differential, especially if there are signs of capsular or vascular invasion. Other possible diagnoses include Hurthle cell adenoma and papillary carcinoma. Fine-needle aspiration biopsy helps confirm the diagnosis. Prognosis for benign adenomas is excellent with regular monitoring, while suspected malignancies require surgical intervention.
