VASAviation - Indeed, very similar scenario to Omaha. ATC didn’t give them much margin though, clearing them to land with one behind on a 2 mile final gives them less than a minute from clearance to vacate. That puts an urgency on the crew to keep the speed up right to the end, and if the braking action at the end is less than expected, a runoff is highly likely. Perhaps when it’s icy there needs to be increased landing separation to give people time to vacate more safely.

Braking action is generally reported fr the main area where the aircraft are slowing on the runway. It can be very good there but the end of the runway can still be a sheet of glassy ice. Close following aircraft and the lead one has to maintain a little more speed to get to end of runway and turn off in time. Not slow enough for the ic on the portion that doesn't get as hot from the braking of rapid deceleration. He had appx 40 seconds from wheels touching on the runway to when he needed to be OFF the runway when the following plane is only 2 miles behind. Controller's fault for trying to pack in the landings so tightly. Pilot's fault for not telling the controller he was unable to safely make the indicated turn in time with the runway condition.

Seems like the landing with long rollouts takes you to end of untreated runways infrequently used with black ice easily formed. You could space arrivals farther apart and exiting 1000 feet prior to end of runway. High movement areas keeps ice forming slower. A/C Speed must be lower for icy conditions after touchdown rollout turns off of runway.

Lets force the pilot to hurry under runway icing conditions. 2 miles behind is VERY close spacing. Appx 40 seconds.... That means the seconds aircraft's wheels were about ready to touch when they called for go-around.

I tow airliners at my work and i can tell you when we cross runways they are usually dry as they get treated. As for the rest of the taxiways, they are not as good as the runways in winter and are usually slippery after the plows come through. So its not surprising you come off a runway with good traction onto a taxiway that has poor traction

not sure what protocol is for ops on departure end of runways in use for landing but looks like the trend would be skidding off the end of the runway because of clearance to exit at one of the last turn offs - as a semi experienced arm chair pilot im sure they do something on their roll out when they know they are going to be going down the entire runway - especially in this case with company behind them im sure crew wants to vacate the runway asap

@@vanessaruiz4705 They'll lighten the aircraft as much as possible. Probably unload the passengers and baggage at the point where it left the runway and bus them to the terminal. Then carefully tow the aircraft back onto the taxiway and to the maintenance area for a thorough inspection of the gear and related systems.

When runways are chosen to land into the wind and there's snow on the ground, the far end is almost always going to be slippery. The wind will blow snow onto the pavement from the incident wind direction. That's going to make those areas wet/slippery/icy regardless of what is falling or not falling from the sky. It's not the fault of the pilots, but anyone whose driven on open roads in winter when the wind is blowing can relate to this simple fact.

16 years flying in/out of O’Hare B637/B757/A320 and 4R is NOT a runway to have trailing aircraft that close behind! It’s always an exit at the end and other than in dry conditions, it must be assumed to be slippery due to surface markings and rubber deposits. The aircrew should have not been pressured nor accepted the responsibility to exit quickly.

I think what didn't help is that they reported the braking action good, sequenced a heavy right behind them and told them to exit at V. They probably had more than enough space to slow down and turn off but they would have kept the speed up and maybe attempted to turn off with a bit too much pace.

Two incidents in two days suggest that the end of the runways need better de-icing as I think was mentioned after the SW skid. I agree longer aircraft seperation should be considered by ATC in these conditions especially as the ends of runways do seem prone to skidding in these conditions. Glad no one injured & well done to ATC for quick reactions.

I kind of agree. On two occasions the controller told the pilot of the very tight traffic in-trail.....you know, basically hinting at no delay getting off the runway. Not the best of situations to be hinting that a pilot needs to expedite. Especially in a 737-900 that apparently already needs a little extra attention on landings due to its higher landing speeds (so I’m told). I’m not disrespecting the controllers as I am one as well, but let’s face it these pilots were pressured atleast a little to expedite in poor conditions.

Incoming aircraft were likely sent around and back to the Approach controller (who would likely put them in holding patterns) until one of 3 things happened: 1. The aircraft had to divert due to low fuel 2. The airport opened up another runway for arrivals that is long enough to handle all arrivals 3. The airport got the aircraft that slid off the end of 4R towed to a place where it wouldn't impede normal airport operations (could be a gate, but I think it's more likely to be a maintenance hangar for a check of the landing gear) The check of the landing gear would be needed because even though sliding off the end of the runway at relatively low speed into soft terrain probably won't damage anything, it will still probably jam dirt into a lot of places it shouldn't be, like the brake rotors and on the inside of the rims. Maybe just going to the gate and having the gate crew pressure-wash the gear would be enough, maybe. I don't know enough to know which option would be preferred, so I'd recommend the safer option of having the gear checked out by the maintenance department.

OK, not a pilot or controller, but my observation... would it not be better to slow down (delay) arrivals in these conditions to give pilots more time for the rollout and vacate the runway? The faster pace seems more conducive to accidents like this and not only cost the airport and airlines money, but end up closing the runway and delaying everyone anyhow. Probably more delay when this happens than if ATC had just slowed everything down to begin with. Am I missing something?

I have often wonder how exactly they get the plane back onto the pavement after they slide off like this? Do they have vehicles specially designed for this or do they just get a couple heavy duty tow trucks to winch the plane out of the dirt? I know they can’t use a standard tug because they are not designed to go off road, especially in these conditions.

Honestly I see no blame that can be attributed to the pilots at this point. It's winter and quite possible the end points of the runways are not getting enough treatment. Was funny that the one pilot reported "braking good" but sometimes it's a matter of just hitting the wrong spot.Kudos to VASAviation for the quick reporting. O'Hare has the most aircraft traffic of any airport in the world. When it's busy there are aircraft landing every thirty seconds at the airport. Having enough runways to handle this level of traffic, as well as having the need to close runways for maintenance means that there are going to be more runways than most airports.

Breaking action on the runway was fine which is what was being talked about. However the areas which are not as well treated by the deicing equipment can be much more slippery. Think of it like hitting a patch of black ice when driving, once you're on it, your a passenger and the vehicle is going to carry on in the direction it's momentum takes it.

That's two aircraft doing the same thing in 4 days. You think maybe it's time to pay the controllers and get things back to normal before someone dies. Not faulting the controller in either case but the same situation at two major airports is a bit much. Omaha after southwest went off the airport inspection revealed no good braking action. Last call there was that the 737 8 minutes before had reported good braking action. Think some people dont have there heads in the job cause of the situation at home
