From Dr. Rosenblum, "The patient did not actually have cholecystitis but rather passive congestion due to elevated central venous pressure from tamponade physiology as blood filled the pericardium due to the proximal aortic aneurysm (mimicking acute cholecystitis). I have seen this once before when a patient had a subacute VSD after a missed MI." Thanks for the great question! -MB
