Do you bring both 719k and 719ke together if your an entry level or just 719ke?

If I check yes on one of the questions and I take a medication that will likely cause issues with the coast guard, do you know if I need to fill out this form 719K(10 page medical form) AND the 179K/E (5 page short form) or do I only need to fill out the longer form? The longer 10 page one has all of the information as the 719K/E (5 page one) plus more information for Dr to fill out such as a breakdowns of the medications/medical issues in addition to all the "physical" stuff like vision, hearing, etc. I just don't know if I need to get both filled out at my Doctor or just the longer one, and it is kind of unclear online. I know for sure my doctor will need to explain why I take said medications and I don't want my application to get an AI status.

It depends what you are applying for. If you are going for an original OUPV or Master's you will check the box that says "Deck" in the upper left hand side of the section.

Who is the “medical practitioner” though? Can it be your normal primary doctor? Say, your primary from the VA? Or do you have to find a medical practitioner specific to this exam for the USCG?
