Isolated right varicocele is rare. And if occurs, we usually do a sonography or CT to rule out a secondary cause like some lymph node or mass. If no secondary cause, it can be treated with embolization or surgery

No. That just means that the plexus is probably stretched from years. And that's why little prominent. But as long as there is no reflux on valsalva- pain improves and semen parameters also improve in most patients.
