Exactly. Even if he didn't trust them with taking off from the intersection again (I woudn't either), they could have easily take off from Mike. It's basically an equivalent of sending misbeahving child to the corner. :-)

I was thinking that exact same thing. When I saw them originally entering at GOLF I immediately saw where the problem was. I just love how the ATC said "Stop! Stop!" and the pilot just casually turns off the runway and says sorry about that. ATC - "no problem" (because no one just died doofus).

OK he's responsible for the potentially fatal error, but the layout and procedures were an accident waiting to happen. Many airports that have a taxiway leading into a two-runway intersection have a firm ATC rule that they never clear anyone to take off from intersections like that, because those are a long- and well-known danger of wrong runway takeoff. It's an issue that has been the subject of much investigation, conclusion and regulation in many countries over many years. In particular, see the 2006 crash of a Comair CRJ100 at Lexington, Kentucky, USA. In the last 10-15 years, various airports have collectively spent tens of millions $$ changing layout to eliminate "asking for trouble" intersections like that.

The tower controller is required to file an incident report which would have been sent to the CASA YCBR district office almost immediately as it's a violation of a clearance in a primary control zone. The crew definitely would have had to file an incident report with their own airline management and this report will work it's way up through the Safety Manager, fleet manager, standards group and eventually the Chief Pilot. Tea and bikkies in the Chief Pilots office would be considered obligatory. I will put a link to the evetual ATSB report for all you know it alls.

Point: that intersection is ripe for confusion!!! Should not ever be used for takeoff. And pilots should brief the potential for confusion and be extra super cautious and check aircraft heading instruments!! That procedure should be done for all runways: verify aircraft heading with assigned runway numbers. Ret Capt

Controllers make mistakes, too. Actually, it is fortunate for the flying public that anyone would want to take that job. Also, airplane pilot is a high-risk vocation, up there with high-tension lineman, logger, commercial fisherman. Much higher than policeman (half of whose deaths and injuries come from ordinary traffic accidents of the type all vehicle operators are exposed to). So, we're lucky to have pilots, too.

By the time the pilot apologizes the excitement and adrenaline surge are well over and everyone is getting on with their day. The 'no problem' indicates we're all glad the mistake was caught in a timely manner, everyone reacted accordingly and we still have the rest of our shift/flight to finish, including now, the extra paperwork.

Joking aside, the comments don't seem to realize how big of a mistake this was. If they used reduced thrust (which they did) and continued the roll, there could've been a big possibility that they would've ended up off the end of the runway, especially with an intersection departure. However, I am no expert, I wasn't there and I didn't look at the airport charts, but it's a fact that mistakes like these kill people. Good job ATC

forgottenfamily ...Intersection departures are interesting and useful, to expedite traffic flow. But, you have to keep your eyeballs peeled to catch the infrequent mistakes. That intersection, can be confusing, but with more due vigilance, the incident could have ben prevented.

Could this error have been caused by a combination of the taxiway being at an intersection and a mid-runway take-off? Doesn't it make sense to taxi further down to avoid confusion?

I live in Canberra and use the airport a lot. That intersection is clearly marked as the ATR's use it a lot. Don't know why he would line up on the Rwy he did which is to short for that type. Bit of a weird one to call.

Maybe the controller had him depart at the intersection because he was a smaller aircraft? The next aircraft in line was a Qantas so it's probably larger than that ATR. Sometimes they do that for wake turbulence separation.

Anthony George nobody ever wants an ATR in front of a jet... they would’ve requested intersection G and the controller will taxy them there. In this case, the ATR was quick enough to get airborne and not delay the 737 behind. Otherwise the order would be flipped. Wake turb doesn’t exist between these aircraft either

We are. Some don't do it in rush unfortunately. Also there are modern systems which tell us what runway we are on or approaching to.

When I saw distant airport lights appear to be correctly dimmed and distorted through the cockpit windows of the ATR, I was convinced.

Runway 35 is 3283m (10771ft) and 45m wide (147ft) plus blast shoulders to take it to 60m (197ft). Runway 30 is 1670m (5479ft) and 45m wide. From where the aircraft began its takeoff roll to the grass at the end of 30 is exactly 1000m (3280ft).

And in general there is no difference between JFK or somewhere where is not so much traffic. There's the same danger and it would be the same damage that can happen in worst case. Sure they could talk about the incidents, but as I mentioned before, it looks like the Americans are just waiting for such things. PS. It's not more extreme just because it happened somewhere in great America ;)

Hey, it's my city's airport! They do intersection departures all the time for ATRs and Dash-8s. Never usually a problem. Good thing the controller caught it though because I can tell you right now - 30 a really, really short runway that usually only the little craft (like Cessnas etc) use. The north-east end points straight at a very large bridge/highway overpass. If this guy had actually got to rotate, that could have been very ugly.

Using taxiway G for this was just asking for it. No clear markings for which runway is which. That’s why you take off from the beginning of the runway, not the middle.

There's a huge difference between lax and professional fluidity. The controller saw the error immediately halted it and provided clear instructions to the slightly confused pilot without adding more stress to the cockpit. Would the controller have achieved a better result by yelling and screaming at the pilot, nope, just increases the risk of further errors. Sounds as if the controller has to deal with low experience GA pilots and trainees as well.

Sounded like there was an aircraft departing so they just sent them to the end instead of having them wait at the intersection for it to depart (or having the departing aircraft wait for the ATR).

there was an incident at the old HK airport I think, an SIA B747, taxing in foul weather, rain with storm approaching that eventually line up and took off from a wrong runway, one parallel to the correct one. This runway supposed to be closed, with construction vehicles at the end of it. There was sadly a few casualty. Pilot found to be at fault, never flew with the airline again but the Airport got away scott free. sad really...

Not only was he on the wrong runway, he was half way along it. They would have struggled to get airborne before the end of the runway. Looks like it was dark, but even so, that's not good. Both aircrew apparently failed to recognise the error. The cross strip is long enough for ATR and Dash-8 provided that they start at the end of it. The Virgin terminal is at the northern end of the terminal complex. I'm actually surprised that they allow aircraft to take off on runway 35 from the cross strip. The Dash-8 aircraft take off from further south but they also start from the Qantas terminal at the southern end of the terminal complex.

VH-VPJ is an ATR72-600, not a 72-212A, although the -600 is based on the -212A design.

Taxiway G should be gotten rid of. Pilot should check runway heading, but it's too easy to screw that up right there. A robust system dictates it should be more difficult to do what the pilot did.

I think taxiway G is mostly used by aircraft landing on 35. There's enough runway south of the cross strip for an ATR or Dash-8 (or smaller aircraft) to exit at G. It saves a lot of fuel.

No excuse for at ATPL holder. It's very easy - check the heading before takeofff roll with intended runway. Even student pilots know to do this.

You cant always be heads up and professional for 10.000 hours. There are gonna be bad days, and those are the ones when stuff like this happens.

They do intersection departures all the time at Canberra with ATRs and Dash-8s. Never (usually) a problem. I'm guessing the pilot didn't do the Canberra shuttle run often.

"Position and hold" has been deprecated (since September 30, 2010) in favor of "line up and wait", which is the international phraseology.

It seems an odd way to call it out but I'm pretty sure he's ordering a right turn to 020 after take off. That should probably be made more clearly but I guess they assume nobody would mistake it for a ground direction. Though this crew did just line up on the wrong runway so extra caution might be advised.

When you takeoff they give a direction of flight they want you to go. Most is runway heading but in this case get clear of the obstacles and turn immediately to heading.

Apart from the main report, why is any aircraft lining up halfway down any runway? Of course people claim it doesn't matter if the aircraft type can easily get off in a short length, but it is always good practice to give yourself as much length as possible.

Looking at a map of the airport, it's easy to tell which runway is which. Sitting in a cockpit at an intersection, it's a lot harder than you'd think. Really easy to make a mistake, especially at night. Certainly, they should have looked at the DG (Directional Gyro) to see if they were on the correct runway but I'd put a fair share of the blame on the basic design of the airport.

Before I soloed my CFI made sure I watched the NTSB video about an accident involving a commercial flight that did this very thing and crashed. The lesson was clear - check that the DG and compass matches the runway heading.

I'm not sure if I'd call that a 'no problem' situation... just imagine if some vehicle had tried to cross RW 30 at the same time... 😕 On the other hand - there will be for sure enough paperwork on that incident and making a fuss about it on frequency does not help in that situation either. Btw, short question: Due to the fact that they aborted the takeoff, when is it necessary to do a brake check afterwards?

Comair Flight 5191 was not so fortunate. The controller was working solo and had to get his paperwork done before the end of his shift no matter how much traffic he had. So he had his back to the airport during the crucial minute the airplane took off.

line up checks .... Compass, heading indicator, if full length, runway markings. so easy to miss the runway.
