Unfortunately I forgot to record at the beginning but here's what happened:
1. Start the run in 2P. (This way drop rate will be ~2x.) It is optional to have a second player since there is not much to do for the second player anyways. I use controller for 1P and keyboard for 2P.
2. Pick up both max health on 1P and have 2P sacrifice max health to give 1P Back Buckler (to keep 1P alive) and Antivoid Combo Strike (to deal damage with only stomping against hordes).
3. 1P chooses Bomb Rod as weapon (it is important that it is melee, it can break walls, and has a large block hitbox: the larger the better).
4. 1P starts playing and 2P can either die or optionally also play the game but leave all power ups etc to 1P. However, it is more optimal for 2P to take some damage or die so that health items can start dropping; the downsides of dying is that it will take an additional health item to first revive 2P before recovering health for 1P.
General strategy:
1. Use stomp and block to avoid damage as much as possible; do not actually use the melee weapon except for finishing isolated stunned enemies. You can in principle block all damage except fire damage on the ground. Deflecting ammos through blocking can also be used to finish off single void enemies.
2. With the two initial power ups you can already clear a lot of challenges easily, but the bosses will very likely remain hard. Focus on getting these power ups/upgrades (roughly in that order): trap dodger/fire/electric/ice/poison stomp, beacons, protectus gem/back buckler (with a sufficiently large block you can easily avoid all damage), arachnid haste, buttons (and leg-button), boots, coffee, combo stomp, spirit insurance, companion, shoplifter/discount. Bullet modifiers/rounds can be useful to combine with beacon/buttons. Wind stomp and similar can be used to stun void enemies. Sprinting icons, reloads, peppers are useless and can be sacrificed. Avoid Jimbot due to the anti-synergy with the block strat. Knockbacks are also better avoided due to the anti-synergy with beacons and buttons. Get as many power ups as possible before doing the bosses to make them die quicker.
3. On floor 1 especially, generally avoid void chunks in the wall until you get less useful things to sacrifice than max health (a second weapon, useless powerups above, etc).
4. Try to keep at least 3 health on you to avoid getting one shot.
5. 2P can also hold a second unused weapon to sacrifice for more useful things later (48:09).
The same strategy can be used for other runs at other difficulties too but this is the hardest thing in this game.
