Walking a Newbie through reconstituting Tirzepatide and Semaglutide

From my research reconstituting like this can damage the peptides. You are not supposed to just depress and shoot/spray the syringe of bac water into the vial of powder. Once again, from what I ready you should be dripping the bac water slowly down the side of the vial into the powder to avoid damaging the peptides. Any thoughts on this?

Generally, the most sensitive peptide is Mots-c and for that one, pharmacists do recommend a slow reconstituting process, but the other peptides are usually safe regardless of the technique used.

As a nurse, who reconstitutes medicines every day… for a one month supply of dosages, I recommend reconstituting 20mg with 2 mL of sterile water. Then divide that into four doses/four syringes (0.5mL). If you’re using insulin syringes, this will be in “units”. 1 mL = 100 units. 0.5 mL = 50 units. 20 mg of Tirzepatide in 4 equal doses will equal out to 5mg (per 0.5 mL) doses. If you’re brand new to these injections, you should start with 2.5 mg dosages for a month or two then make your way up to 5mg. Starting at a low dose helps your body get used to the peptide, metabolize the peptide and prevent getting sick. You would have to order 10mg vials to get a 2.5 mg dose after injecting 2 mL of sterile water.

yes, but typically our patients are being dosed in "units" like 15 units, so there are 6(six) 15-unit doses in 1mL. Check with your prescribing provider to clarify.

Two things I see that are incorrect per the peptides company I order from. A) when reconstituting, the water should be slowly injected into the vial and aimed at the glass sides of the vial, not directly at the peptide. Then the mixture should be slowly, gently rolled (the vial on its side, rolled back and forth), and absolutely not shaken. B) Only enough should be reconstituted for immediate use. Tirzepatide is not to be 'stored' for later use, as some peptides can be. And Tirzepatide is not to be frozen (for later use) like some other peptides are. I'm surprised that a supposedly professional business would either not know or leave out this information.

I have been looking for this info!! If one follows the prescription model of 2.4 a week for 4 weeks, you would be dumping out ALOT of the product if it came in a 5/10/15 mg dry dose!!! If you want to keep it simple, add 1 mL BAC to 10 mg or 1.5 mL BAC to 15 mg. Therefore, you will have concentration of 10 mg/mL in both situations. Lastly, if the dose you want to take is 2.5 mg, then you would draw (assuming you are going to use a standard 1mL insulin syringe that has 100 unit total marks = 1 mL) 25 units (0.25mL). If your dose is 5 mg, then draw 50 units (0.5 mL). Search for reconstitution calculators online as well to double check your math and what random people on the internet tell you to do!

no!! Every time you use a syringe it dulls the needle and using it over and over can cause scarring

Most of the vials I have seen that require reconstituting are usually only 3ml. Yours may be a 5ml capacity vial though 🤷‍♀️. Just to be safe I would use no more than 3ml in the vial. But a 1:1 ratio would mean you are pulling 100 units for 1mg. I’m not sure if you have Reta or Tirz but the according to the clinical trials Eli Lilly is doing for Reta recommending dosage for Reta begins at 2mg and dosage increases are done in 2mg increments…therefore a 1:1 ratio would be 200units which is A LOT to inject at once. A more ideal recommendation would be 1ml of BAC water to your 5mg vial and for every mg you need you would pull 20 units. So 1mg=20units, 2mg=40units, 3mg=60units, 4mg=80units and 5mg=100units (which would be the entire contents of the vial). For Tirz. 2.5mg is the initial recommended dosage and the dosage increases are 2.5mg increments according to Eli Lilly (the owner of Mounjaro). Therefore 1ml of BAC water to 5mg vial would be 2.5mg=50units and there would be 2 dosages in the vial. Hope this helps! 😉

OMG😳 NEVER inject directly onto the peptides, they are fragile, tilt the vial and slowly inject the water onto the side of the vial as to not wash the peptides apart.

Semaglutide and Tirzepatide are made with long chain amino acids/peptides. Those are powders so they have to be mixed. That is all Eli Lily and Nordisk does before putting the mixture into those pens.

Surely! It would be ideal to ask the pharmacist this question; our pharmacist usually says our medications are stable for 30-90 days from reconstitution, on average. Please keep your un-re-constituted peptide in the freezer until ready to re-constitute it, as that will keep it viable for months.

Great observation. Different medications: Semaglutide vs. Tirzepatide. The concentrations are different in the small and medium vials (2mg/mL) of Semaglutide compared to the large vial of Tirzepatide (10 mg/mL). Not only are they different medications, but we are also limited by the size of the vials - the dosing is accurately accounting for this.

It also makes a difference as to the recommended dosage you are trying to draw from the vial. Using certain amounts of reconstitution solution can make it easier to draw out the desired dosage. {Example: For a 40mg vial of Tirz. I personally use 1.6ml of BAC water… (that is a 25mg/ml concentration) every 2.5mg would be 10units. My desired dosage is 10mg…so I would pull 40units and this would give me 4 total doses in my vial}.
