That sack is actually used in the Game Over screen, where it drops from the top of the screen, and you can play gold to revive yourself, rather than starting from the beginning of the stage.

9:16 I need to point out here, that the game has no knowledge of the fact that the "camera" is being moved. It's a feature of the Dolphin emulator that allows you to see what's currently being rendered. To the game, the camera's position is always stuck there stiff and the only way to legittimely move it is by moving Wario in this case. It's not like the lens flare effect is dependant on Wario's position. If one were to modify the addresses of the original camera position, we might see that not to be the case.
