Amitriptyline was approved over half a century ago as an antidepressant and still remains popular today. Interestingly amitriptyline provides a similar degree of relief from depression as more recent additions to our armamentarium. Selecting among the various options involves considering other circumstances: medical issues, patient preference and side effect profile.
Over the years, the indications for amitriptyline expanded far beyond depression and often without either formal study or specific FDA authorization. Among the conditions routinely treated with amitriptyline are fibromyalgia, chronic fatigue, diabetic neuropathy, pain lingering after shingles, insomnia and low back pain. Amitriptyline ranks among the least expensive options to prevent migraine headache and may even remedy chronic itchiness.
A major issue with amitriptyline involves tiredness or fatigue. For this reason most doctors recommend taking the medicine just before retiring for the evening. In fact many physicians prescribe low doses to treat insomnia rather than larger amounts necessary to combat depression.
Among the many undesirable side effects of amitriptyline are dry mouth, blurred vision, constipation, weight gain, difficulty urinating and erectile dysfunction. Problems concentrating and the potential for cardiovascular issues pose further concerns. Common to all antidepressants, the FDA mandates a warning of increased suicidal thoughts and behaviors especially among those less than age 24.
Amitriptyline interacts with a significant list of other drugs. Special cautions concern triptans used to treat acute migraine attacks, Lyrica, gabapentin, tramadol and anti-psychotics now commonly employed in treating schizophrenia, mania of bipolar disorder and agitation associated with autism in children. MAO inhibitors must not be used within 2 weeks of amitriptyline. Overlap with the popular SSRIs and SNRIs such as Prozac, Celexa, Lexapro and Zoloft may risk a nasty condition known as the serotonin syndrome. Consuming alcohol while taking amitriptyline poses further risks.
After a course of therapy the dose must be gradually tapered rather than abruptly discontinued. Among the antidepressants, overdose of amitriptyline poses the greatest risk of death.
When used appropriately, amitriptyline continues to offer significant relief for a number of conditions of uncertain origin. Unfortunately the scientific underpinnings of how we select our therapies lags far behind our confidence level and ability to write prescriptions.

I’ve taken 10mg for about 8 years. I had daily tension headaches, and sleeping issues. This has helped a lot with both. But I do have vivid nightmares as soon as I go to sleep, wake up after 10 minutes, then sleep through the whole night fine. Weird. I’m sure when I try to stop, there will be withdrawal symptoms. Best take medical advice before stopping.

This guy is amazing. I learned so much! I have been taking Sertraline (50mg/day in morning) since last summer, and started Amitriptyline (25mg/day at night) about a week ago. My sleep has been great, and NO migraine headaches!!! Which is why it was prescribed. I am definitely lethargic the next day, but it isn't bad. I already have low blood pressure to begin with, and it has exacerbated that, but I'm already used to dealing with it, so I can be prepared. I no longer have allergies either! So for me, so far, the side effects haven't deterred me. I remember I did have some side effects when I first began Zoloft, but they eventually went away. So I am thinking they will eventually go away from Amitriptyline too. It has been a game changer for me!

Last year I developed panic attacks and insomnia for the first time in my life (I was 35). It became so bad, I would get a panic attack every time I would lie down. Nothing worked; zoplicone, Zoloft, lexapro, mirtazapine, lorazepam, clonazepam, trazadone, seroquel, olanzapine, Effexor, then along came Amitriptyline. Anxiety gone. Panic attacks gone. Insomnia gone. It was the only thing that worked. I take 30mgs at night, and will probably do so long term. If you are suffering from anxiety and insomnia, I would say at least look into Amitriptyline. A lot of doctors were hesitant to prescribe it because of the side effect profile; I met an psychiatrist at the hospital and she said “sure, let’s try it. It’s proven effective…” I can’t thank her enough. She saved my life

I've been using/testing Amitriptyline for about 6 years now. Just one small dose (20mg) before bed to treat chronic foot and ankle pain. Its paired with Gabapentin for adequate pain relief and both are essential for results. I find that 20mg before bed will "zombify" me the entire next day. What that means is that I'll never feel fully awake or clear-minded. It makes me a very boring person socially with nothing to say. Just puts me on auto-pilot. For this reason, I don't take the Amitriptyline on nights before days off because I want to feel alive on my free days. It works. Most people take higher doses to treat an entirely different condition (depression). I think my brain would explode at those higher doses. Some days I feel like I blew up my brain with just 20mg the night before. BUT everyone is different so maybe that's just me. Either way, I've tested these things out personally and this is the data I've collected based on my experience over the years. I'm 32 years old now.

This morning I woke up ok-ish. I expected grogginess after trying gabapentin (sp?) and other drugs, but after being awake a while, I am getting dizzy and when I lay down there’s a throbbing head ache. What’s sad is that my neck, face and shoulders don’t hurt as much. There’s a numbness there and it feels so nice… It’s so sad when you have to choose between life’s pain and sideffects. It's gotten rid of my nerve pain (that took about 2 plus months) and it's helped me to drop 15lbs in weight so far. I'm not looking to come off of this medication anytime soon. @@lisad4854 I wouldn't. Watch "The History of Psychiatry" or listen to Dr Peter Breggin about the harm these drugs cause. I think our government wants us on these drugs. I don't like it but you have to make your own choice. I can tell you I am not depressed but each and every single visit to the Dr I am offered a psych drug. I'm so tired of it. I told my Dr I will never take them and he still offers them. I have lists.... Also these drugs are re-labeled black box. So many names. Be careful and vigilant because even if they say these drugs are not addictive they usually are and they often cause brain damage. You have to study deep to find the true meaning of these drugs. Now you tell me who's crazy because I could never hurt another with these drugs. I take Xanax. For 30 years now. I wish I'd never started but when I was young we weren't told or had any real way of knowing the side effects. At one time I was given 6 Xanax 1 mg and 600 mgs seroquel for sleep. I lost years of my life. My health and my children seeing me in such a drunken state daily. A pancreas attack. GB removal. My health has been ruined by these drugs. Been researching for so many years. I advocate against these drugs but I leave the choice to others except babies, children and teenagers. These groups are all targeted.

Now, I am still on 75 mg and it’s not working like it did before. Going to go a couple more weeks on 75 mg and if it’s still not working like I need it to -and as it used to - I will be contacting my doctor to see what else can I take. The amazing thing for me was that when I first started taking it - it truly changed my life! 100% of pain gone! It was hard to believe and there are no words for the gratitude I felt. I had my life back after having spent 12 years in pain, with numbness/electrical like sensations in my hands and legs. I suffered with daily chronic migraines for 10yrs which I had to inject myself with Imigran twice a day. They stole my life as the migraines were totally debilitating and it stole my children’s mother from them as I was bed bound most days. I was referred to a pain specialist as I also suffered with chronic pain in my spine due to my spine being fused and a badly tethered spinal cord. He put me on 50mgs of amitriptyline each night and it completely changed my life! I’ve been taking it now for nearly 5yrs and I can honestly say I’ve only had about 4 very very mild migraines in this whole period! It’s been life changing and a miracle drug for me 🙏🏼 as a bonus it also really helped my nerve pain from my damaged spinal cord. I’ve recently had a nervous breakdown due to something really traumatic and having ptsd. My psychiatrist consultant unfortunately can’t put me on any of the SSRI antidepressants because of the risk of serotonin syndrome mixed with the amitriptyline. She put me on mirtazapine but was horrific side effects and I was taken off it two weeks later. They are not sure if it’s because it was mixed with the amitriptyline? So now they are trying to up my dose of amitriptyline by an extra 10mgs each night working up to a total from the 50mgs I’m already on to 75mgs for my ptsd symptoms and depression. Only I’m really really struggling with exhaustion the next day and not sure I can continue but if I remember right I felt the same way when I first started the 50mgs and it levelled out eventually.. the only other option is for my dr to wean me off all my amitriptyline totally and start me on a SSRI but there’s no way I am prepared to come off it as it’s an absolute God send regarding migraines prevention and nerve pain. I highly recommend it if you suffer with either! Absolutely brilliant review thank you 😊

I appreciate what you’re doing discussing and getting the proper information out there for people. But this drug in particular needs to be further evaluated especially when it comes to withdrawal. I’ve been trying to ween off 5mg for the past year and a half. It has completely destroyed me mentally and completely ruined my GI tract. It’s a shame that this violent withdrawal isn’t talked about. @@NirajPoudyal hi, i dont know your age but, if you are under age 65 (and also aged above 18), from the leaflet: "The usual dose of amitriptyline for depression in adults is 50mg to 100mg a day". 10mg is way too low, i dont see how that would help, apart from placebo effect. i think low doses like that are either used for older people eg. aged 70+, or if using amitriptyline for something else such as a pain medication. so if u want to see if amitriptyline works for you on depression and anxiety, you will need to go up to the correct dose range. i did start out with around 20mg and then 40mg.. that didnt do anything. 50mg started to feel like it was working but still not enough, so then for me 75mg was perfect to successfully treat my depression and anxiety. sadly i am no longer taking it as it caused me tachycardia. this is a common side effect unfortunately. so i ended up needing to switch to something else, so now im taking vortioxetine 15mg. its too early for me to know if vortioxetine will be good enough for me, but so far its ok; im feeling ok so hopefully it will stay that way. wishing you luck

If you have come across this video and you have GERD/GORD/NERD and IBS please beg your doctor for about 100-150mg of amitryptaline it’s been a god send! I have sleep apnea from ibs stomach pains and constant stomach acid from gerd and I’ve struggled for three years now and finally I was put on 100mgs and it’s been a life saver. Kills pain in 30 minutes and I can sleep now for a change. And I’ve stopped my fimotodine Tablets. And I’d tried them all ranitidine lanzoprazole emeprozol esomeprozol pantoprazole etc etc and they’ve all failed!

Thank You for sharing! This was a lot more information than I got from my doctor! I think I'll skip this medicine - only 10 mg. daily though, as I prefer to stay slim at all costs. Of course I wasn't told that it will mean weight gain. I like to know what I am putting in my mouth and how that will work in order to make an informed choice for myself. Then I will know what is worse, the condition I am trying to treat, or the side effects of the medicine. Thank You! I will subscribe to you.

I am on this for depression and sleep problems and I just hate taking it. It was too heavy for me and i could not wake up in the mornings. I am extra tired and it causes such vivid dreams i wake up with my heart racing. I also have to take it very slow when i wake up because I would wake up drowsy and feel dizzy. If anything it makes me feel terrible overall. Doc told me to halve it but it still makes me feel messed up. Depending on your health it might cause too many symptoms in some people.

You are right, (elavil) Amytryptaline is the standard off all antidepressants, its pretty safe but you will have sleepiness, this drug is what every antidepressant is compared to...... even to this day, every drug has to be at least 20 percent, well never mind to technical.....
