I never had these until I lost weight. I look older now than before. So the old addage is correct: fat fills out wrinkles. I am okay with that. I wear my gray hair and skin creases like a badge. I earned it.

Same here . As long as my body is healthy, I am Ok with gray hair and a few creases on my face - I feel liberated not hiding my grays and creases 😂😂😂

I have noticed ( now that I’m 68 ) that many people who lose weight when they are older, lose fat in their faces. I keep telling my female sibs, who are in their 50’s and trying to keep a very slim figure, that they do need fat on faces. They seem to prefer being very slim to looking younger. In my case, I finally gained weight in my fifties because I had osteoporosis. It took a few years but I gained 20 lbs. It has made a huge difference to my face. But I glad to read that so many women do not care about the wrinkles, etc. I shows we love ourselves just as we are. ☺️♥️

I have lost 84 pounds and my once full face has taken a hit. I feel it’s almost melting downwards. I’m a smiler and don’t mind my smile lines but I now have the marionette lines and a bit of a turned down mouth. I look tired when I feel the best I have for years. Thanks for a great explanation.

When I was a kid nobody cared. The elderly was just that, the elderly. Now it seems to be a crime to have aging lines and wrinkles. If all of these procedures were free to everyone then by all means go ahead and make everyone feel some type of way for having signs of aging. We live in artificial times. Eat right, exercise and keep a healthy attitude. Have hobbies and interests. Don't sweat the small stuff.

@@cameliap1146 They had the spot light in their own way. It was rare to put emphases on the looks of aging back then. Young folks are getting things done as well. Lots of things done to their natural looks. Folks can't even make a decent expression anymore. Walking around looking like mannequins these days.

I used to not have super noticeable marionette lines but getting masseter botox made them PROMINENT. Not to mention it messed up my smile too and the corners of my mouth don’t go up anymore. I’m hoping it’ll all go back to normal soon 😔

Gosh that is concerning. Thanks for sharing that info. I've also heard radiotherapy can backfire too as it removes fat from the face if done too deeply, which is aging.

Great video 👍🏼 I’ve always wondered what that shadow was that was freaking me out. I don’t have the lines but I have full cheeks and this video explained it perfectly. You can tell you are an artist by the way you speak about the face and shadows in your videos

You explained this so well, thank you so much. I don't have marionette lines as such but I do have slightly downturning on my mouth corners. I have found facial massage very helpful. I am concerned that lipo and also radio frequency would lessen fullness in the face, which is, in itself, also aging.

The model Paulina Porizkova has a great video on KZbin about the invisibility of older women. She points out that real people have lines and movement in their faces. It’s called expression and is a critical part of communication. The only people over the age of twelve without them, have nothing to say.

She is more visible at 60 than most other women at 20 due to her still astounding beauty. She just feel invisible relative to her more explosive visibility in her youth. However much we get plastic surgery we can't even touch the corner of her natural beauty. Just browse plastic surgery pictures and who can replicate her jawline, her cheekbone, her eyeshape, and most importantly her goddang height? She can turn head just by standing still. Anyway, most people are invisible anyway. If you are old and you still want to be visible, become a politician.

the imperfection exists only in your heads. i love an old worn in face. there's so much to see and the substance and warmth of humanity exists in these old faces. i have written off tom cruise as an actor because he might look the same but he can't move his face so he can't act anymore. Cruise now just looks like a caricature of his youngster self. Embrace age. Become human again. You really want to help your face, eat vegetables daily, drink ginger tea, hydrate, love people and animals. That'll get rid of those fkn marionette lines you're so concerned about. also ... stop looking at the mirror so much.

Oh please . I have more than enough with eating vegetables, lot of water, ginger the & co. Nothing stop ageing. So many gurus of aerobics/ healthy eating & co. are flabby or like a dried raisin. Genetics comes first. Eat what you want in small portion and do what you please. And if you want and afford it , why not plastic surgeries. Medicine is our servant. I see it like that. And no. I think that people now look in the mirror way too much and see imperfections that are really just normal aging and they've been 'trained' to see this as something to be fixed. My opinion only.

I have these and they are pronounced. I hate them and hope to get them fixed in the near future. Thanks for this video because I have seen another doctor that also spoke of not using a lot of fillers in this area. Most docs want to do lots of fillers and I now understand how that can make it worse.

I greatly agree. Filler is always only masking a problem, except for the indication of volume loss. It's the only legitimate purpose of filler to fix the volume loss.

He well explains what is happening beneath the skin. A face lift just pulls the skin up and back, but the under process goes on. Later you may need another face lift. There are some faces that don't really get jowls, but wrinkle, take your pick. To be sure it is all aging though. Best to undertreat as he says, for sure.

I was thinking about that too. Both smooth pdo threads for collagen build and cog lifting threads for a more immediate lift. Isnt that a good option?

@@liset4081 yes it’s way better option than going under the knife & anesthesia for a face lift. It’s also cheaper safer & there’s basically no down time. It’s a no brainer 👌🏼

Yes, but you have to do the theeads every year. It doesnt last long. And for what you will spend every year on threads, you might as well do the face surgery which lasts 10 years or more and its one time pain vs yearly

“All the laxity caused by time and gravity . . .” That took my breath. I paused. I looked into the mirror that I have been purposely avoiding. I scream, Yes! Yes! I am a candidate for a full and complete facelift! Then, I reflect on the reasoning: to have a better appearance for myself, so that when I do walk past the mirror that I have been avoiding, the dog will no longer attack my shadow. Educationally, this video is inspiring. I accept myself in this present moment, and at this present time. And if by chance, money reigns, or rains, down upon me, I know where my next flight is taking me. Perhaps that’s next year. This master of aesthetic facial creation, the doctor - is taking the time to explain multiple types of procedures, the facial musculature, and other topic matter. He has a wry sense of humor which is followed by a dead-pan look into the camera. Hilarious. I would love a sculpted face - uplifted, and tightened with no rigidity in its form. Then, I smile, and think of a future when the dog stops attacking my shadow. Funny, I thought. Nota bene: Financing. I am comforted in thought. No. No. I pause: Audi A8 versus face lift. Which one? One day. . .maybe both. Reality sets in. Priority becomes apparent. Future goals . . . perhaps a flight, procedure, and vacation wrapped in one. I will be circulating this video to my many friends whose dogs also attack their shadows. Then, I will not feel so alone. Who knows, maybe my friends and I will get together and make changes for the next stage of life, before we are bed-ridden. Good grief. Tempus fugit. Do it now. Do not delay. Be happy now. In this moment. Whatever you look like and however you feel, changes can always be made, in heart, mind, body, and soul. And then . . . there is always a call to this doc’s office. Next year, I say.

I had a deep plane face lift and fat grafting 6 months ago. This didn't make either nasal labia folds or marionette lines go away. Both are fairly deep. I'm having more fat grafting to my lips done in 2 weeks. Now I'm considering a little fat grafting to the nasal labia fold and marionette lines at the same time. Naturally I'm not going to get another face lift now, but still want to get rid of those lines. I've learned a bit about the anatomy in those areas from your video and understand why those lines form and why fat grafting there may remove the lines, but give the jaw a heavy look. Since I have a relatively small jaw, perhaps get a very moderate amount of fat grafting, to at least soften the lines a bit, without squaring my jaw too much. I would welcome your input, if you think this is a bad idea. Thank you for explaining thinks so clearly in layman's terms. This is exactly the kind of information I need in order to make informed decisions! You now have a new subscriber!

Thanks. I would have to see you in person to offer the insight you seek. In my experience, the best facelifts are superb for the neck, great for the jawline, good for marionette lines, and ok for nasolabial folds.

@@jonathanzelkenmd Thank you for your quick, helpful response. My face-lift did all the things you described, except the marionette lines had almost no improvement. The nasal labial folds did improve a little bit. Except for that, I'm perfectly happy with the results. Don't think it was a failing of my surgeons skill, because I should also point out that I was 72 years old when I got a deep plane face & neck lift, upper and lower bleths, fat grafting, brow lift, chin implant, and fat added to my lips. I look at least 15 yrs younger. If I can get the 2 problem areas improved, I think that will give me another 5 years restoration. 🙂 My surgeon did such a great job. Made me feel beautiful again. Love him for that! And love you for sharing your knowledge and enthusiasm for your craft with us! Your Before and After photos speak to your extraordinary skill. By far the best age restorations I've seen in the many, many photo from other surgeons websites I've visited. I've seen some Comments here complaining about the fees, but the best results and skills are seldom inexpensive. So worth it. Best wishes for your continued success!

Ofc it is. It makes people look sad. Like glabelar wrinkles. Other wrinklers under eyes and mouth are normal, smiling. But those two makes people look in certain emotinal way.

It's not conventionally a beauty standard. People just want to revert back to an acceptable standard. That's why they don't appreciate the aging process. Youth features will always rein supreme against mature features

This is a great video that explains the why and how's, and I like how you show the various effects by using the tape on your face. Nice job!! Seems like the skin starts looking more mature in a different way when you insert anything therefore I try to do it all with exercise and she has helped sooo much. I hope it helps you 💕

The best exercise for the face is doing plank pose everyday. It tightens the jaw, neck and shoulders. Hold for several minutes. You will see results. Give it a try☺️

I have a "baby shape" face and noticed these even when I was a teenager, they are still subtle now I'm in my early thirties. one doctor told me botox makes it worse in time, and one told me it helps prevent it worsening , what is your opinion? thank you

I do not like botox around the mouth in general. Sometimes, when you Botox the muscles that pull the corners of your lips down, it can soften the shadows somewhat but it’s not my favorite solution. Not risky, per se, just not likely to erase those lines.

I’ve had them since I was a young kid myself and I’m now 41. They definitely get worse. I personally liked getting Botox in my DAO’s but it doesn’t last long AT ALL and of Course it doesn’t fully take the problem Away

Sunken tear trough and marionette lines, now I know what these little SOB are on my face. I noticed them after I started having teeth issues. They've gotten worse.

U R Awesome at this explanation of MY issue with my face... im59, black female... I wanna do this for myself. That lipo looked brutal, though it's probably MY best option! I'm in great shape, but.....these jowls 🙀I've Never had a single surgery in my life! I'm scared.... should I do this???

The radio frequency skin tightening solution sounds the least invasive, though the lower lift seems like a close second in preferable adjustment method. I have small puffed perioral mounds while having a somewhat oval baby face, genetics I suppose, while I’m physically very slender. I look very young for my age in general. I have high defined cheekbones, but with the lower subtle mounds and I sometimes think my face shape from the front sometimes looks weird like a slightly bloated/subtle lower rectangle. My lower jawline looks different, somewhat sharper, at an angle compared to frontal view. Maybe partly a food allergy or something. When I was a kid, my lower face shape changed quite a bit at different ages, and was on and off more angular with a higher defined lower-jawline and other times more oval/puffier. It’s really annoying to me. I wouldn’t want the mounds turning into lines down there so I observe them closely on occasion and strive to take care of myself as much as possible. Unsure if it’s better to do something about it sooner before it drops to that point.

Wow at age 56 , I’ve now learned I have all three issues described here , sunk-in tear troughs , nasal labia folds and marionette lines .. I hope to find a Surgeon as well educated as you in my area to address these aging issues of mine .

Hi. My ex in Hollywood is a LONG TIME good friend of Gwen Stefani. We all work in " the industry " both music and film. She has had several procedures. The word on the street so to speak, is that her label warned her. And it had something to do with her insurance. Singers vocals can be affected by too much lip surgery. And nose probably too. So think what you want, but its common knowledge that she's becoming " addicted " to surgery. That pillow face thing is pretty prominent without makeup but we'll never see her without makeup- ever. Lets see in 5 years if she still looks like a human or a robot.

From my experience No. I have marionette lines due to chubby cheeks. Got buccal fat pad removal. It only caused my face to look much more aged.😭😭😭 like hallow cheeks but at the same time chubby cheeks from the area where the marionette lines are... and i still have those annoying marionette lines. Maybe im thinking about getting liposuction near that area. Buccal fat pad removal was a huge waste of my money and I dont like how I look.

Hi doc! Great channel!! What would you recommend for someone having the first signs if sagging? Like subtle nasolabial folds and jowling starting to appear from excess facial fat on the cheeks and jaw? Squaring of the face so to say. Who has thick skin/ mid 30s. Facetite?

There are many options. For nasolabial folds we may begin with injectables at the base of the nose to soften the folds. For jowling, Facetite may be a great option for someone in her 30s.

Buccal fat removal is one component of lower facial sculpting. I would call this surgery lower facial sculpting as a catch all phrase.

This is a great question. In the early healing period there can be a sense of tightness or pulling, especially in the neck, but the sensation subsides with time.

I had a face (MACS) lift, but that perioral mound of fat wasn't lifted, which in hind sight makes sense to me, because that fat pad wasn't removed. What do you think?

I think MACS lifts are inherently limited and liposuction + ligament release is an important adjunct.
