warning to all who are considering a double all-on-four... zirconium on zirconium is VERY clacky... they told me I'd get used to it, but I never will... I HATE it. And zirconia doesn't look nearly as good as acrylic for some reason either... everything I read before I did this said the opposite... Overall though, having all my teeth extracted and doing this procedure was one of the best decision I've ever made... I waited a year (instead of the usual 4-6 months) to get my final teeth.. just to make SURE the implants were fully integrated... for those who are interested, mine cost $48k... my case was not complex in as much as I didn't need any grafting, sinus lift etc...

Thanks Keith for your comments. Zirconia is a white ceramic metal and on upper and lower jaws do sound like to ceramic plates banging together. This not usually the best option. Upper Zirconia for cosmetic and strength with lower one-piece milled PMMA is considered an excellent option however the zirconia will wear the lower teeth faster due to the strength of zirconia vs PMMA. IT's best to talk with your dentist and dental technician on what are the options and the best for longevity and comfort. And ask what they would have in their mouth! Zirconia may be the strongest with the best longevity however the comfort level is poor due to weight and noise. I appreciate your comments.

There are limitations to all the prosthetic options. I prefer delivering zirconium. Acrylic will look like trash after a few years and some people need constant repairs.

There is pros and cons to all treatment and even bridges have problems as the teeth are joined and if there is an issue with one - e.g. porcelain chipping, long-term decay, etc - this will impact all the teeth in the bridge. If an implant fails, it only involves one "tooth" and generally not the adjacent teeth. Pls discuss the options with dentist and consider 2nd opinions. All cases are different and the risk vary per case. Someone will have higher implants risks than another and the same with porcelain bridges. Hope this help. Take care

I had top & bottom All on 4 implants fitted in 2015 and I have to admit - I'm less than happy with the results. I feel that the width of the plates in narrower than what I originally had, as well as there being less teeth (10 on top & bottom) meaning there are gaps at the back of my gum line. As a result cosmetically my face has become sullen & sunken cheek making me look older than I am. You cant even see my top teeth unless I do a fake "cheesy smile" - (sort of defeats the purpose of getting them) Also due to the change of width and the build up of material to make the plates, plus the gap between plate & gum line I now have speech problems and an annoying lisp which I didn't have before. As time goes by the gums recede more and the gap increases with the posts starting to show more and food is constantly getting stuck there. Main problem also is as a result of these changes - narrower width, less teeth at back, gaps etc my bite isn't aligned properly causing me to sometimes grind my teeth as well as bite down wrong - causing 5 teeth to break off and then having to wait 2-3 months for an appointment. For $41,000 I would have expected a lot better

(I find that quite a worry cos you'd think a dentist would be concerned that a broken tooth could cause more damage to the other teeth it comes in contact with - so would at least want to see me to be sure). I have my doubts over the quality of the material used & that its not what I paid for. Also wanting to charge me first n then treat me!!! Is there somehow a way to get them checked? Ive also asked on previous visits if there's a way of fixing/extending thee gum line further to reduce the gap thats getting bigger between the plate n gums n was told tbat there isnt a way. @@glebeboi There are different materials used for All On Four bridges - cast frames, titanium frames, acrylic teeth (of various brands), zirconia monolith, zirconia frame with porcelain teeth etc. - each has their pros and cons. For most systems you can add to the gum line to reduce the gap - it can be harder with some systems but not impossible.

Re Gum - yes that's what I figure as well however I feel that they dont want to do it thereby saying it cant be done saving them the expense of doing so. How or what can be done to correct my speech that's been altered by these implants as well as the facial changes. Could I possibly get new ones made wider (or where they shouldve been to start with) and with additional teeth put at the back where there are now gaps?

I’ve had it done with no problems. You get what you pay for. It’s a costly procedure - “if it sounds too good to be true” , it usually is. You HAVE to go to a reputable implant place.

Implant position can be corrected with angled multi unit abutments, usually 15 or 30 degrees. What is disturbing is to see all on 4’s completed with CBCT scan and still the surgeon is creating a full thickness mucoperiosteal flap.

Thanks for your comments Implant Dr. Implant position can be correct with angled multi-unit abutments however there is a limit to this correction if the implant is placed incorrectly especially in cases where the vertical dimension is not ideal. Not all systems allow for this correction. Although CBCT allows for options such as guided surgery, soft tissue management (to gain keratinised tissue) around the implants - and thus longevity - does not always allow for flap-less surgery. Although a personal preference, most experienced AO4 surgeons opt for full thickness flaps to fully visualise the surgical field.

When he put the screw on the upper left he can't screw the right. I got 6 xray to find out why but je cant screw all so I came back a week later , he tried to screw it same thing but after 2 xray he kept trying to screw and xray everytime he screw tight the left side I felt so much pain in my gums , my cranium felt like exploding but he stil cotinue to screw the right but the pain is in the left. Final xray has gap and he told me to come.back after 2 weeks he will screw it one more time. My question is : is this normal? i feel that something is very wrong but I am not sure AND AS LONG AS I CAN'T say the exact word or problem the franchise dental company kept denying things.

@@guaranteedallauthentic6074 You either have an overcontoured prosthesis and the discomfort will quickly subside as your bone and gums remodel or you don't have a passive appliance and you're being set up for big trouble. Implants won’t withstand a large, continuous force in a non-axial direction for long. It's not difficult to diagnose but definitely return to your clinician to advise. I’m puzzled why he/she doesn’t just pick up the abutment that isn’t seating in the mouth. It’s critical you have four of the six radiographically seated and that the occlusion is good before you proceed with picking up in the mouth. If it’s not a completely passive prosthesis, you’re potentially looking at long term complications.

Your teeth can start crumbling the first year if they are acrylic. There are certain zirconium that will start the third year. I found a page funded by dentist who last reported in November, 2018. It’s called Clinician’s Report. It only took We a week of searching to get to seeing it because I didn’t know it was available. It reviews acrylic and three to four zirconium styles. I wish I had known this before I wasted $36,000. Then, find out if the lab is any good though I don’t know how to do that.

Hi Sheila - there are many options for FINAL bridges with this treatment. Currently the best overall option is a solid titanium frame with acrylic teeth as the titanium can be made immediately after surgery and the final bridge can be completed within a few days. Zirconia bridges have some advantages with strength however they cannot be made at time of surgery and cannot be modified easily if there some gum changes (e.g. shrinkage) in the future. There is also the problem with Zirconia against Zirconia (upper and lower) noise and aggressive wear of natural teeth against zirconia if only one arch is completed. It's best to way up all the options and find a dental AND technician that you feel comfortable with and can discuss all the options.

No.... We have a large window behind me and it's tinted with a white opaque tint. So it's natural sunlight on the background and just some small LED lights on the front. Simple set up that we do for videos and patients to see their new smiles.

Brilliant advice! That's exactly what I was thinking... I'm sure this is going to sound like a stupid question but why would I not just put an implant for every tooth that has fallen out? Why would I choose all on 4 if they are just going to sit "attached" or "on top" of the gum? When you pull the lips back won't the line connecting the all on four with the natural gum line show when I smile? Won't food collect in that gap between the gum line and the implant? Doesn't it make more sense and look a lot more natural if I just place an implant for the tooth that has fallen out, keeping my natural gums, and thereby also leaving a natural "space" between the teeth as opposed to these all on fours that are very obviously one single structure rather than 12 individual teeth? I'm struggling to see the logic of an all on four as opposed to individual implants. Your expert opinion would be very highly appreciated, Cheers!

Sad that no-one replied to this sooner. When a tooth is extracted, the bone starts to be remodelled as part of the healing process - it shrinks. There is often not enough bone left to support an implant - ideally we want 12mm height, 6mm width and 6mm thickness. This bone loss happens quickly if 2 or more neighbouring teeth are removed, leaving too little bone to hold an implant in some cases only a year or two later. Remember as well that most people who need multiple implants have lost their teeth through periodontitis which destroys bone. In such cases I'm happy to find enough bone for 4 implants although All-on -Four is really not ideal in any case. Sometimes there might only be enough bone for 2 implants in the whole jaw, a situation I´ll find at least once a year. Bone grafting is also not the solution. It's very patient and situation dependent - sometimes it works, sometimes it doesn't. Implant success is about 90% guaranteed first time (don't believe the manufacturers´ claims of 97 - 98%) but nobody releases figures on bone graft success in dentistry.........

My dentist said that he could even drill on my nasal bone :( will the gap between my gum and the acrylic gum will get bigger and that could make it more difficult to clean and then cost infections ? I think dental implants are just a waste of money. They say you can still get gums disease if you don't get your implants cleaned routinely etc. And other problems. And if the bridge breaks you gotta pay $$$$$$ to get a new one but then what do u use while u wait for a new one. And I've heard you can still have bone loss with implants. So I think the "promotes bone growth" thing is a lie.

what is the normal procedure if there should be a mishap on an "All on 4", like one of the front top teeth should break away from the denture moulding. (On average) How long would it take for an appt to be made and how long until I would get my arch back?

It depends on the material that the bridge has been made (e.g. denture acrylic or PMMA (graphene) milled etc). Normally 3-6 hours in most cases. The issue is finding the cause of the fracture and to remember that this a prosthetic replacement of teeth and that there is less feeling (proprioception) via the bridge on implants in bone over teeth in the jaw with ligaments and nerves inside the teeth.

How many teeth have broken? The timelines that you have mentioned seem excessive however surgical and repair protocols vary depending on the clinician and laboratory. A temporary bridge or plate can we made if this time impacts daily routine. I hope this helps

Hi Doc , I have all 4 maxilla , the final bridge doesnt fit well. When my dentist screw the right side, he can not screw the left amd vice versa. Had 3 xrays but still he can't figure out what was wrong. Then after a week , I came back , he tried to screw the 3 left (extremely painful that thwybhave to give me nolotil medicine ) and the 3 right then xray but it shows gap. He then tried to screw one more round on the right and one more PAINFUL round screwing on the left and then xray again. I still shows gap on the left and almost none on the right. The left which has gap on the xray the flange is digging into my gums painfully. While on the right which shows almost no gap in the xray , the intaglio is not even touching my gums. I just got all on four and the guy placing my implants did not position the bracket correctly. My top appliance was 3/8 off center from my original bottom teeth. This made it not possible for me to get my final new teeth. I paid 19000.00 up front. What do I do now? 3 month visit was May 15 2019 I'm scheduled for a wax model the 29th. Can you help me... please

Implant angles can he corrected via changing the abutments but there is only a 30 degree correctional change. So the options are (in order of complexity) 1. Change the abutments to new angles 2. Keep as is or improve as much as possible with option 1 but have the screw hole in a compromised position. 3. Replace the implant to the most ideal position. Naturally the option depend on how severe the problem.

When they tell you it will only be mild discomfort for a few days they are lying! If they told the truth it would discourage a lot of people from getting them. Just hoping for the best!

The implants should last the patients life with excellent oral hygiene - the teeth or bridge will last dependant on the material. Acrylic teeth around 5+ years, titanium hybrid 5-7+ with an simple replacement, or zirconia bridge 15+ years. Hope this helps.

all on 4 or 5 or 6 people dont get fooled by promises ,and expectations every case is different and results vary from patient to patient ,complications are more common then non

Dabi, this is complex treatment and there are always pros and cons to any treatment. The more complex the treatment, the more complex possible complications. Every patients needs to do their own research and discuss whether these options are suitable and if the benefits outweigh the risks and if required, talk with both several medical/dental professionals and people that have had the treatment. There are no medical procedures without risk and a patient must be fully informed at all times. Thanks for your comments and for highlighting this.

Thanks for your comment John. Failures from implant treatment is rare if the clinician is experienced. Implant surgery is one of the most complicated dental procedures and with this type of treatment, planning and an excellent support team is critical.

4 months after surgery all in 4 i have infection of one maybe two implants.i have controlled diabetes 2 is this normal and what to do

Hi Haris - Sorry to hear about your problems. If there is infection around the implants after 4 months, it's critical they are removed as soon as possible to limit the amount of bone loss. If there is already extensive bone loss, a bone graft can be placed when these failed implants are removed. Unfortunately, the bridge cannot be place back on less than 4 implants. In some cases, if there is enough bone after implant/infection removal, immediate placement of a new implant is possible and the case can be immediately fixed. Haris, in each case the revision surgery is different but the initial response - removal of the infection and implant - is the same. Hope this helps.

I have developed ONJ after having an All on 4 done. The oral surgeon is loathe to remove the implants as he fears the sites will not heal. I have a lot of exposed bone and it seems to have stopped growing larger, there is absolutely no sign of of healing. So, after having spent a heap of money because of my vanity and reluctance to have a regular denture, I am now walking around with no lower teeth - just four implants with little silver caps on them. Any suggestions???

Procedure was done Feb/17 and denture was placed immediately. No problems evident at 3 months. When I went to have impressions for permanent denture, a small unhealed area was present, so temporary left in place. Several months later, I developed a severe infection and the unhealed area had expanded. I have seen the oral surgeon {university based) several times and he still cannot decide what to do. I am quite angry as the fact that I had been on bisphosphonates for years and was on Prolia at the time of the procedure was not addressed by the dentist or anesthetist.

His, I suggest an OPG (or better a cone beam) to check the bone loss around each implant. If the bone loss is > 20% (after 12 months) either grafting and/or burying (gum closure) or implant removal with grafting may be best - each case needs to be assess individually. The current protocol is a fasted serum beta cross laps test (CTx) to determine your bone turnover. If above 200 should be okay for healing and implants. Fully normal bone turnover is 400+. If under 200, I would suggest careful monitoring of the healing. Hope this helps.

Well none are perfect. Dentures aren't perfect. Never have been never will be. Snap in detures. Are not perfect. Over denture all on 4 3 D bone scan. All of it. None is perfect. Trying to figure out what to do is almost exausting. Listing to every dental post. Oh here's a good one dentist love to to. They love to advertise what implants will cost. They advertise that they will let you know the price of dentures and you molants. The price is the bullshit false advertising

Hi Lance. You are correct. The is no perfect solution. Each patent is different - their situation (time, finances, location, etc), health, anatomy, etc. However, you are doing your research and educating yourself. Find someone that you feel comfortable with and discuss the pros and cons for YOUR individual situation. Thanks for watch and commenting. I feel your comments are common for many people. Take care
