What I feel is, when dr strange said "the problem is you trying to live two different lives" In spiderman NWH trailer, I guess the spell miss fired and split spiderman/Peter's timeline into two. One where everyone knows he is spiderman and the other one where nobody knows and he can stay with MJ. Just like what the ancient one did with dr strange.

Moreover the ancient one attacked dr strange with the spell which left ash Marks on his chest and in spiderman dr strange did not hit parker with the spell. The ancient ones spell was more yellow/orange like the normal magic and the one dr strange cast in the basement was more purple/red chaos/dark magic. We never know. It can be one timeline split into two or a portal has really opened to multiverse.

Time works differently in time machine ( made by iron man ) & time stone. Agar aap time machine use karte ho to aap future Change nahi kar sakte aur past mein Nexus event ban jata hai. And time stone se aap past mein jaa sakte ho aur past mein changes karke Future Change ho sakta ( as shown in doctor strange whole sanctum sintorum was repaired using time stone.
