He hears a legend about the Blue Spider Lily and finds the doctor (ik he wasn’t alive at this time but hey different story) who is working in the experimental medicine. He forces the doctor to give it to him and he kills the doctor once his demonic attributes awaken. He then does what Muzan does: create demons to find the Blue Spider Lily or find one that can withstand the sun”. Simple as that.

Yoriichi acknowledged that muzan could have killed him if his attacks landed, but muzan sucks in actual combat since he doesn’t do more than merely overpowering most of his opponents. I would think Muzan would be able to take control Koko's movement just like what he did with Nakime. He instantly blew her head unless Muzan is severely weaken due to the drug Tamayo gave.
