If you remember that when Akaza first appears he literally is a few cm away from crushing tanjiro head and then at the last second rengoku saves him so tanjiro would die as shinobi would be unable to block the attack. She would just stab him poisoning Akaza but tanjiro still dies then Akaza leaves because he did his mission.

Wait, would not that be even worse? Akaza wont eat her and thats shinobus strongest and only venom to take out upper moons. Tanjiro was easy pickings and inosuke could not do much either (he froze sensing how easy akaza can take him out even while fighting a hashira). So at best he would kill those two and leave or at worst obey orders and see shinobu as a hashira, not a woman and take her out as well since she could only slow him down
