Maybe que caused a civil war, and made the side that kept the focus tree lose. Then changed the ideology back to normal.

Ideology would need to be changed before that civil war if you wanted the original leader to be in the new country, not afterwards. If the two civil war sides have the same ideology then your current leader stays with the original country while the leader for the new country just gets a generic name and portrait.
