Try infecting more people at the start or wait until the population density goes up. You could infect an army and then make that country declare war on another country. Trust me it will spread. But not destroy an empire only weaken

I think it's funny that you started with not knowing the correct date of the American Civil War, but by minute 8 you were making observations about the plague that took epidemiologists decades to work out. Yes, the observation of plague infections rising as the population goes up, then falling as people die, allowing population to rise again, is exactly how many epidemics have gone throughout history, before modern medicine. It's how the plague that actually most closely matches your timeline, the Colombian Exchange's introduction of smallpox to the Americas, went, until the numbers of indigenous populations fell too much for it to spread further.
