ITB syndrom is no longer considered an injury caused by friction, its caused by compression of a highly inervated fat pad on the later aspect of the knee. Also, the ITB does not snap over the lateral epicondyle. Instead, the ITB has an anterior part and a posterior part. Tension transfers to the posterior part during flexion, giving the illusion of snapping.

Hi Thomas, athletes are prone to ITBS but if you are not an athlete, there are still some traits that give you a slightly above-average chance of getting ITBS. In most cases, the IT band gets irritated or swollen from rubbing against your hip or knee bones when it gets too tense. One of the symptoms is clicking sensations, you might feel a snap, pop, or click on the outside of your knee, which causes inflammation.

@@EastCoastPodiatry On the contrary, the "rubbing/friction" - theory has been refuted. I can provide evidence if u want. Also, after rehabbing (exercise therapy) for ITBS the ITB actually get more tense! I also have evidence for this. If ITBS was caused by tension, this would cause more pain.

@@thomasroth1991 I am interested in your findings. I tried exercice and shockwave but my pain is only getting higher. I'd like to understand the process of itb pain and how to cure it.

​@@thomasroth1991not sure if u lack common sense, but the compression is literally what's causing the friction 🤦🏽‍♂️🤦🏽‍♂️🤦🏽‍♂️ How can you not understand that simple? Furthermore, u have been going on abt "evidence" yet fail to quote a single meta analysis. A laughable reply would be cherrypicking a study.

This is exactly what I have and I have been diagnosed with a leg length discrepancy in the past and I recently had a problem with plantar fasciitis. Thanks for sharing.

I am not a young person... i am not a runner or a cyclist. I get IT Band pain. I experience a dull pain on the lateral side of the legs regularly.

laparoscopic surgery fixes it once & for all. The shockwave just causes more fibrosis, shortening & pain.

Does the itbs band run down the inner thigh to the knee also?

Hi Pcg, the IT band starts at the hip and extends to the outer side of the shinbone, just below the knee joint. It does not run down the inner thigh.

Is pain only at the Gerdy Tubercle insertion (from overuse) but not higher up considered i.e. an early stage of ITBS, and does it (typically) have a different prognostic, better/worse, or different requirement for activity modification?

My itbs pain only shows up during the middle part running. When walking or taking the stairs it doesn't show up.

How long does this injury last? Because I think I have ITBS and it's been a month since I injured my knee. Also why do I feel severe pain when I stand up from a sitting position? Thank you in advance!

does it hurt when you go down stairs? if yes, thats itbs. i suffered for it for years. have a professional PT check it. you may also do home remedies once the pain is gone probably after 2 weeks. strengthening your hips (glute medius) 1. clam shell exercises first there is progression in strengthening but i suggest check it with a qualified PT. im not a fan of gel/ultrasouns and shochwave. did these and it didnt fix my knees. i fixed it by strengthening exercises.

I have this problem right now, it’s 4/half week, the pain is on the hip joint but mainly on the left hand side of the outer knee. It hurts really bad at night and stiff in the mornings. I have problems climbing steps and sitting up and down like you. I’ve also been on naproxen&paracetamol for almost 2 weeks no improvement and doing stretches and strength exercises for ITB in the last few weeks. Are you better now, how long did it take to recover and what did you do to help in the end? Thanks

Hi, I have this syndrome for 4 months now. Doctors cannot seem to give me a clear solution for this. First one I saw told me that only cortocoid injection unde the tendon will help. I once exercised with a physiotherapist, but the inflamation got so high I could not walk for a week. Now I did one shockwave session - he did radial shockwave but only focused on one point , as you did for the focal one yourself- do you think the physiotherapist made à mistake ? Now 3rd day after the session me knees are sore and inflamation getting worse. All information I read says I should feel pain only locally and only in the first days. I live in Paris , and cannot seem to find a good specialist to help me. Thank you

@DD-sx9tq hi, I am not sure the shock wave therapy was applied correctly in my case. He just put it on my sore part and did not move arround. What helped me is change the shoes or the soles and not make much effort with my knees. Now I am 2 years in and I don't have pain. But I have nit tried to hike again, so I guess the final results will be known once I can do hikes for 2 or 3 days at least.
