In this episode I discuss what are the best treatments for pulmonary fibrosis depending on most common situations. It is important to determine the cause of the lung scarring in each individual case, because depending on the situation:
1) no treatment may be necessary if the cause can be removed (occupational lung disease, environmental exposures) and the fibrosis is stable over time
2) a treatment for a known cause or lung inflammation can be given if this is driving the fibrosis (e.g. if the fibrosis is caused by another condition affecting the body - typically inflammatory conditions such as rheumatoid arthritis, scleroderma, other autoimmune diseases or inflammatory lung conditions such as chronic hypersensitivity pneumonitis)
3) antifibrotic therapy may be recommended. There are two indications for antifibrotic treatment: 1. IPF (idiopathic pulmonary fibrosis, or progressive lung scarring of an unknown cause), where two medications are approved (pirfenidone and nintedanib) and 2. PF-ILD (progressive fibrosing interstitial lung disease, also known as PPF - progressive pulmonary fibrosis). In PF-ILD, there is usually a known cause for the fibrosis, but the condition cannot be called IPF. Antifibrotic treatment with nintedanib is available for PF-ILD regardless of the cause of the lung scarring if, over time, there is worsening of respiratory symptoms, lung function and chest CT radiological images.
