I have cousins who are half Italian and are pretty much fluent, and they say it's extremely difficult to understand Italian dialect. I discussed this with them in the context of the HBO show My Brilliant Friend, which is set in Naples, and they found it extremely difficult to understand Neopolitan, which is also a Southern Italian dialect - Pugliese is from Apulia, which is the opposite side to Naples, but pretty much on the same latitude. So even if you speak Italian, having someone talk at you in a quick flow of language can be very difficult. I don't think they were the only people to miss that he said "mamma mia!".
