I worked for Brown & Root for 10 years as a High-Pressure Boilermaker Foreman! The reason mostly was errors on the process! Like they would tell us to cut into a gas refinery tank, after the engineers told us it's been drained then filled with water! Well guess what they never drain the tank and it's full of gas or whatever chemical they had in them! BOOM! I knew those guys and they got burned bad from the flash! 1975 to 1985 era Houston and Pasadena

Do not rely on other engineers, before working on any vessel that contains inflammable product, check the vessel yourself to see to see that any inflammable product remaining in the vessel has been purged, said vessel filled with an inert gas like halon .

Thanks for sharing your experience. The modern burner are equiped with flame eye to minimalize explosion when start up the burner, when igniter failed ignite the burner, fuel valve will automatically closed and cobustion air still flowing to purge the burner.

that's true, but fire eye failure can allow the ignition to proceed and the explosion risk remains. many of the more modern designs have dual fire eyes or temperature probes to ensure ingniter or pilots are lit before main burner gas valve is allowed to open

Actually the reason for any boiler explosion is simple.........the design limitations were exceeded resulting in catastrophic dis-integration of the boiler. You're right though, I've never been involved in a serious boiler explosion but I have seen a few minor ones. 1. The operator was in a hurry and decided it would be ok to bypass most of the purge cycle. Nice BANG but no damage. 2. The boiler was fired by liquid fuel, some of which had accumulated in the bottom below the tubes. The was more of a WHOOMP than an actual explosion but it occurred a few minutes after a successful start. 3. Conversion of a boiler from liquid fuel to gas. Long exhaust stack and short purge cycle. Again a nice BANG, sort of like a shotgun, but no damage. 4. This wasn't a boiler, it was a thermal oxidizer. Its purpose is to heat noxious plant gas to 1600F for 11 seconds or more. This would burn the noxious odor. After the first start, all went well but a couple of minutes after shutdown there was a very loud BANG followed by a fairly loud hiss. The plant has hydrogen at 1400 PSI, there were only 3 of us present at the time. We thought a hydrogen line had exploded so we ran as fast as we could to the evacuation point. What actually happened was the plant gas valve was pneumatically operated butterfly type and the shipping pin was still in so it didn't close completely and allowed plant gas to continue entering the combustion chamber. When the gas built up to the insulation spikes (which were still red-hot), it ignited. The hiss was that the explosion blew the front part of the fan off and severed a 1/2" compressed air line operating at 100 PSI. The bad part is that the control panel is right above the fan and I was standing there about 2 minutes earlier shutting down the unit. Had I not walked away, I'd be about 2' shorter now........ Boilers are generally pretty safe but they're not very forgiving of failure to adhere to established operating procedures. In other words, it doesn't take much of a screw-up to blow stuff up...........

Not much of a screw up? Leaving the shipping pin in on a gas valve is a catastrophic failure in safety measures... It wasn't only design limitations, but user error. There should be mandated sensors to prevent this sort of foolishness. You can explain it away to try and minimize it, but it sounds like whoever installed and/or inspected the thing was beyond negligent. Everything you described is user error and lack of maintenance/being dumb and converting boilers.

@@maskcollector6949 As to the boiler conversion, the era outlawed the use of liquid fuel so we had no choice but to convert to gas. Certainly, the guys who did the conversion were responsible for it but if the era were to not be such bullies who absolutely love to throw their weight around, it wouldn't have happened. They need to cool it a bit. As to the shipping pin......the valve was installed more than a year before the plant started up, leaving it in was the right thing to do but yes, it certainly should have been removed before the last actually statrted up. The main contributing factors here were the fact that there were no stepladders anywhere nearby. The company should know better Plus, the simple fact that the valve was high up and out of reach is a design error. It could have been placed low enough to be accessible from the ground.

My old Boss had a Boiler to go down on him. The boiler had set long enough it had lost all of Steam. He should have brought it up to Temp slowly but no, he Manual set Boiler on High Fire In turn That Dummy Stretch all the tubes Had to be rewilded Insurance Inspected. YEA DUMMY..
