I'm currently writing a paper about females being prone to ACL injuries than men but all this conflicting evidence is sending me into a spiral.

Do trans people who take hormones have different risks of ACL tears as compared to people with the same sex assigned at birth? I feel like case studies about trans people who take hormones could be very insightful for studying the impacts of hormones on the risk of injuries of the knees.

Additionally, it poses a question: does hormonally transitioning change the musculoskeletal systems of trans people significantly enough to increase the risk of injury? Other questions: do trans men who take testosterone, after time, experience less severe symptoms of previously diagnosed musculoskeletal disorders? Is the opposite true for trans women on HRT?

imo the answers to your first two questions are yes, and last two are 'it depends'. Anecdotes from me personally as a trans woman: I went through 28 years of life without any musculoskeletal injuries or disorders. In the 5 years of being on estrogen and a testosterone-blocker, i: lost a whole shoe size, developed mild sciatica and SI Joint dysfunction (as a result of increased anterior pelvic tilt, as a result of loosening of pelvic ligamenture, as a result of hormone therapy - in my opinion - GP and physio agreed), sprained an ankle, fractured a wrist, had a grade 3 AC joint separation, bruise very easily. I mean yeah, maybe i'm just getting older, maybe it's all a coincidence. But I think there's something more at work.

A second point, to echo your call for more trans-inclusive or perhaps even trans-specific research. Just over 2 years ago I was a volunteer in one of the first covid vaccine trials here in the UK. During, I was contacted by the research lead for my area who explained that the 'national institute' behind the programme had emailed out some guidance that affected my participation, and i will copy and paste what they said here because I don't think there's any way of tracing this back, although i'll avoid naming them so it doesn't turn up in any search results: “The non-binary/transgender option has been removed from the ‘sex’ field. Capturing biological sex is important to ensure that we are not enrolling anyone who is pregnant - which is an exclusion criterion. We also need to investigate whether the vaccine reactogenicity, safety and immunogenicity are equivalent in individuals of male and female sex. Additionally, male sex is associated with an increased risk of severe COVID, so biological sex is important in the ascertainment of potentially vaccine enhanced disease. We have to report the study results to the regulator by biological sex.” But this was and still is wrong. The first claim is lie because the participation process was thorough in screening out pregnant people. But the second claim is a heinous justification. There isn't something magical about 'the male sex' that conferred increase risk - it is the hormone profile. I argued this in more depth to the research lead who actually agreed with me. He then forwarded this to the central team. I was kept on the trial under 'female', and the central team at the 'national institute' indicated that they planned to take account of this episode in the design of future trials. I didn't expect to write all of this when i came on to youtube today lol, but I think this stuff is so important. There are so many blindspots in science and medicine, and for something as universal and as powerful as human hormones there is so much we don't know, nor shall we ever if we keep ignoring the spectrum of biological configurations that exist in society!

Given that transgender related cross sex hormone treatments are (legally) restricted to adults, there's virtually no change that can be made to the male or female "hard tissues" i.e. the skeletal structure (specifically the pelvis, the Q angle, the shoulders or the elbow carrying angle).

The fact that the size of ACL in female is substantially smaller male, seems like it has solved the mystery. To show caustion, look for small ACL size group and the odds of them getting injured.
