Just recently found your channel and I love it! I’ve had quite a few scopes and surgeries done and anesthesiologists are always just the sweetest people with their patients. I love that you can have both instant gratification yet have a connection with the patient, and it’s one of the factors in making me consider the field. nice vid, Max. Love that you're doing this and shedding some light on some misconceptions. As a fellow PGY2 anesthesiology res at NYU, I think we there's a lot more we can still talk about, esp regarding how people think we just put people to sleep and then "chill"

Yeah I agree with you 100%, and part of my goal with these videos is to give med students and the public a better sense of what we do as opposed to just pushing prop and trading stocks. CRNAs will NEVER take over the role of an anaesthesiologist, they simply don’t have the medical knowledge or training to take over more complex procedures.

I'm in nursing school so I looked into cRNA before. Not interested in cRNA at all, but from my research anesthesiologists are so in demand in America cRNA or physician anesthesiologist it should not be a concern. cRNAs are getting tons and tons of job offers right out of school easily. Also, it seems like there always needs to be a physician anesthesiologist supervising cRNAs to help with the plan and if a procedure gets complicated.

I was in your shoes 26 years ago and now have been in practice for 21 years. I also did a peds anesthesia fellowship. I have no regrets in choosing the field of anesthesiology. It is a most humbling profession. You will have many moments where the s**t will hit the fan and you just want to panic. Just stay calm and do what you have been trained to do. You will be the controller of how everyone else around you will behave. Just stay in control and never let them see you sweat. Through the years, your calm demeanor under pressure will make everyone trust and respect you. Good luck with your career!

I worked for a doctor who married a dermatologist and he is such a kind man. We had a patient coming in for treatment and she was a frail old lady, her son brought her. She had something going on with her nose and he was concerned. It wasn't anything that was going to impact her treatment he would provide her but he asked her and her son (as her son could better hear and understand his concern) if he could take a photo and pass it on to his wife to look at it. He would then follow up with them if she recommended that this woman comes in to her office for a consult. Sometimes there is benefits when a couple are both doctors...

Well I Chose Anaesthesiology..because it makes you a specialist right away and I thought it is easier to undergo through the residency program and clear the board exam as compared to other full fledged clinical specialities. Besides Anaesthesia makes you a specialist in one field right away while other clinical specialities are more broader.

I've heard that anaesthesiologists have to do a lot of math too, so it would be good for someone who enjoys doing a lot of complex math. Is this true?

It is true that we do a fair amount of math, however I wouldn't describe it as complex. A lot of the math I do is figuring out medication dosing based on patient weight, for example to induce anesthesia with propofol: 2mg/kg of Propofol * 70kg patient = 140mg. Propofol concentration is 10mg/mL, so then I'd give 14mL of it for induction. So, nothing crazy, but these types of calculations come up frequently.

Would you recommend anesthesia for a woman who also has aspirations of being a mom? Do you think it would be possible to balance work and personal life?

It is definitely possible! I know several female anesthesia attendings and residents who have young kids. I can't speak from personal experience, but it likely requires a lot of time management skill to pull it off. But at the end of the day, if you really want something, you will do everything you can to make it happen. If I can interview a female attending who has young kids at home, I think that would be an interesting video!

Hello, first year med student here and I just wanted to say I have really enjoyed your videos! Even though its so early in the game, I do have an interest in anesthesia but was wondering your perspective on the whole CRNA situation and whether you think there is gonna be more of an infringement/encroachment in the field of anesthesiology? In addition, as I know other people in the field, I know that lots of private groups have been taken over by firms and it hasn't always been a good thing. Of course I am going into medicine for reasons besides financial gains, but I was wondering your take on whether, among other things, you think that compensation will still remain high in this field?

Hi Danny, glad you enjoyed the videos and also glad you're thinking early about anesthesiology! Good question about CRNAs and definitely important to be thinking about. So just to zoom out a bit, midlevel practitioners are present in many (most?) specialties, so anesthesia isn't unique in that sense. That's been a trend in medicine for a long time and I'm sure will continue that way. Specifically, the trend is finding less expensive ways to deliver medical care, and mid-level providers help answer that question by doing more of the bread-and-butter work in their respective fields. Going back to anesthesia, that means doing a lot of the ASA 1-2 cases (ie. patients aren't very sick at baseline) and also mostly not doing subspecialty work (eg. cardiac anesthesia, regional, OB, liver transplant etc) or at least not the advanced cases in subspecialties. What that practically means for anesthesiologists is that it's important to figure out a way to distinguish yourself from a CRNA, which for a lot of people means subspecializing. For other people, the answer to that might be getting involved with hospital committees, clinical research etc. Overall, I don't think this is an existential crisis for anesthesiologists, but it is important to make sure your career path builds in reasons for hospitals to hire you as an MD and not replace you with a CRNA. As far as private groups are concerned, I think that more broadly medicine has moved away from small private practices and small academic centers to HUGE private practice groups and HUGE academic practices that have acquired swaths of smaller private groups. I haven't started looking too seriously at the job market because I still don't know what kind of practice I want for myself, but I get the sense that attending jobs just depend on what you want and what kind of contract you work out with the hospital/group that employs you. In regards to compensation, I think it really depends on where you work (bigger cities tend to pay less, private practice tends to pay more) and the details of how much you're willing to work each week.

I have pretty reasonable hours, especially compared to some of my surgical colleagues. On average, I work between 60 and 70 hours a week.

Oh I had a never block for my royally F'ed up shoulder and upper arm. It caused me so much pain I was cussing the anesthesiologist out than he just put me under. If I could have hit him I would have yet he was out of reach. And them boom I was under. 8ish hours later I woke up and the ortho nurse was so nice I mostly forgot about the pain for the nerve block in my shoulder. Plus she game me dilaudid so I was happy. And might have given me more than I should have had in the hour I was in recovery. Yet my ortho was a great ortho. He is ex combat medic. And when he saw the x-ray and ct scan of my shoulder he said to me "I have seen shit in the field yet never seen anyone F up a shoulder like you did." I shattered the ball in the shoulder and broke the Humerus is three places. OI is a bitch. Also broke the ball off my right hip at the same time.

Hey there, I'll answer this question based on education level. FYI there are no anesthesia-specific requirements except for doing at least one anesthesia rotation during medical school. - Highschool: To important a good foundation in the sciences, pay particular attention in biology, chemistry, and physics - College: Take necessary premed classes (gen chem, organic chemistry, physics, biology) and keep in mind that some med school require biochemistry, math, and English. Also, you can major in literally anything as long as you do premed classes in addition. I majored in Philosophy and Neuroscience! - Medical school: During your clinical years of medical school (3rd and 4th year), it's important to do at least a month-long anesthesia rotation, better if you can do a couple. Hope that helps!

Nice video! I'm having some doubts now that I have to choose specialty. I've always liked surgery since I started studying medicine but now that I finished college, I can't find the right surgical specialty for me. All of them have certain aspects that I don't like and have the feeling that some of my knowlegde of medicine will be forgotten over time. Anesthesiology has come to my attention and if I had to go for a medical specialty that would be the one, but I don't know whether I'll miss the possibility of becoming a surgeon, even if I don't find any surgical specialty convincing. Any advice on the matter? :D:D

Hi Leighmielle, there are prerequisite science courses that you need to take to get into medical school (eg. chemistry, physics, biology, organic chemistry). Beyond that, my recommendation is to take any classes that interest you, whether or not they're directly related to medicine. My undergraduate degree is in philosophy which is a field I really enjoyed!

Becouse when im yong in junior high scholl i had saved from Angiofibroma nasofaring juvenile, 23 times go into hospital only for blood transfusion because shock et cause profused epistaksis until high school operation angiofibrome transpalatal. on my jurney in clerkship in medical faculty in surabaya indonesia i had trepanation becouse rhinorhea LCS becouse destruction basis cranii and regretion angiofibrome. God saved me with long jurney as a patient. Now its turned my duty as His toll to give hope and cure to patient.

I am changing my goal as i wanted to be a chemist before realizing it was a pretty bad job for me I was wondering, how much maths do you need to do when doing anesthesiology? I am horrid at math, also, do they teach you the math in medical school?

Some of the math is taught in medical school, but the physiology taught is key. Besides, you learn how to be an Anesthesiologist during residency. You have to get *into* medical school first and make it through.

Ideally yes, but in reality it ends up either being an interpreter on the phone in the OR, or if that's not feasible, then instructions are explained and expectations are set (via an interpreter) in the preoperative area before going to the OR.

Well, if one works without being payed; you lose your dignity. However i saw a program about "middle class americans", that could not afford dental care, never mind cleanings and fillings,: it was like 3 Between 350 and 500 to pull a single tooth. I recently got rid of a wisdom tooth: anaesthesia €15,02 (with quicksleepr) and the extraction itself was €45,22.. oh and a 2 surface white filling? €63,31. My insurance covers everything i want up to €750/year and if Ihave problems paying if I need tp pay more than it covers?i just have a chat with the receptionnist; they can split the payments.
