Some advice/thoughts for other commenters from a fellow MTD sufferer who is finally coming out the other side after several years of recovery. I'm a singer and my MTD came on gradually. At first my voice would occasionally get hoarse and fatigued, and over time it became more frequent. I believe this was caused by a combination of some bad vocal habits (which probably would have remained minor if I didn't use my voice so much) and overuse (singing, vocal teaching and classroom teaching). I felt that something was wrong and I went to an ENT and got diagnosed with MTD. It was great to know that I wasn't just imagining the problem. BUT, knowing I had MTD ended up also hugely escalating the problem because I was constantly worrying about it. I saw two speech pathologists and did all the exercises they set me (which included strengthening exercises and also exercises to relearn how to resonate healthily). I quit my job to take time to heal. I did daily meditation, yoga, speech exercises, eucalyptus steaming and massage. I was being so diligent but I was stressed about my voice every single day. I felt like I didn't know how to talk properly anymore, let alone sing. My constant anxiety about my vocal health ended up being the main hurdle to recovery. In my case, I had to learn to let go of worry and perfectionism. I had to learn to be okay if my voice was fatigued. I had to learn to be okay if my singing voice didn't sound as good as I wanted it to. I had to learn with being okay if my throat felt a bit tense. It was very difficult! I've only felt this shift in mindset in the past 6 months, but already my voice is a lot better; the less I cared about how I sounded, the better it got. I am still healing from this problem but I am so much better than I have been for the past 3 years. Just know that if you're feeling helpless, frustrated or defeated: I've been there! I am very aware of the fact that mindset and emotions greatly affect our voices and therefore, my program includes that part too. I am happy to hear that you are doing well. @@healthyvoicetips Thanks Katarina. I feel like your videos comprehensively cover all elements of recovery. My speech therapy ended up being quite costly and it could be prohibitive for some people, so I love that you are sharing your knowledge on this platform for anyone to utilise. I just enjoy doing the exercises with you! I also love how you emphasise that there is no one path and that all the treatments go hand in hand, eg for a long time I thought vocal rest would be enough, but I actually needed to pair that with focusing more on the corrective exercises. Then I was frustrated when the corrective exercises weren't 100% 'fixing' me, but I needed to change my mindset as well. Love your work. And thanks to you and Christine for helping us and empowering us!

Me also I think. My throat is probably sore all the time but worse after speaking or having to speak loud . I dread phone calls and if I push on my Adam’s apple I can feel pain . 2 years struggling with this. ENT said it was vocal hyperfunction and to get speech and language therapy… he made no big deal about it but it’s a big deal to me! I’ll try these fairly simple exercises and hope for the best. Vocal hyperfunction is a different way to say functional dysphonia so very similar to MTD (even though you may not have that diagnosis). And the solution is voice therapy so I highly recommend finding a good voice therapist. I know that ENTs often downplay the voice issues but I completely understand how difficult vocal problems are! Your problem is real, I get it!

Hi Katarina...I love your videos...I have been dealing with mtd since I was little...as a child of an abusive alcoholic I learned to keep quiet and when I did speak, I was so nervous my voice would be full of tension. It took me some time to understand my disorder as it was "my normal" and I coped with it until adulthood and my vocal load as a teacher reached a tipping point after a combo of laryngitis and a series of traumatic events. I was misdiagnosed and was a "mystery" to most laryngologists as they did not believe that I knew that anxiety and trauma played a large role. I was finally properly diagnosed by a doc who is one of the founders of the spasmodic dysphonia association...not only did he properly diagnose my MTD, he knew exactly which muscles were overactive and over compensating....it was the omohyoid muscles. He proved it by injecting a tiny amount of botox into each omohyoid and in 2 days I had no tension and a clear voice. But I have not done botox since as I don't want that route of using a band aid. Any tips for tension of the omohyoids? I am also starting somatic therapy to try and put my nervous system into a more relaxed state. Thanks so much.

Nice to see more info on this out there. I'm not positive I have MTD but my voice has been sore for at least a decade mainly from just teaching guitar and talking excessively. I have done backup singing in the last few years (completely just ignoring the pain response). I've spent thousands on resonant voice therapy with a pathologist, paid about a thousand bucks total for laryngeal massages, vocal rest and singing lessons to almost no success. Also the ENT have said my chords don't have any polyps/nodes ETC! Very frustrating. Will definitely be all over this channel. Thankyou

Thank you so much for your videos. I had a stroke 4 years ago and developed muscle tension dysphonia. I originally couldn’t speak but speech therapy helped a lot. I found your page last year and the improvements to my voice is remarkable. I still have challenges but my voice has greatly improved. after ENT and endoscopy, couldnt get clear diagnosis from them. 3yrs dealing with MTD, I am confident this is the cause of daily tension and strain.

Dear Katarina! Thank you so much for this video. As you know, I recognise everything you talk about. I've dealt with MTD for many years. In the last few months however I've been getting so much better. Partly from talking to you and applied your knowledge (meditation FTW!) but also by going to vocal massage, and working on my whole body. What also is helping me is strengthening my Transverse Abdominis. I didn't know it was weak, but it was. I had used my Rectus Abdominis instead, creating too much pressure (also RA is a fight or flight-muscle which can trigger parts of our brain to be in a too tense state - thus creating a spiral of becoming more and more tense and scared. I think I told you I became afraid of my won voice. I belive this has been part of that). Finding the trans.abs and use them more efficiently has been…wow! For ME!!! As you say, it’s different for everyone. Thank you again! :-)

Wow, just saw an ENT doc, and that's exactly how I felt. He said everything looked good. He did see evidence of acid reflux damage near my vocal cords, but the vocal cords themselves looked fine. But I know something is wrong, because I have no range, no freedom, in my speaking, like I used to have.

It is a very common scenario what you are describing. Unfortunately! I suggest that you try voice therapy as a trial therapy. If that helps, you know that it is the way to go.

I have MTD caused by acid reflux. My Speech Therapist told me that the muscles tense up and become over-reactive as a guarding mechanism to stop acid getting in your lungs. It's also something to note that reflux can happen while you're asleep, something known as silent reflux, which causes reflux laryngitis (another fun condition that I've dealt with on top of MTD). It's important to get your reflux under control or speech therapy will only be a temporary fix. With MTD it's super important to remove the causative factor to reduce the risk of relapse and damage.

I developed MTD pretty rapidly after catching COVID about 7 months ago. I've been in speech therapy for 3-4 months trying to get my singing back and it feels like I've lost a part of myself. I'm trying really hard to let go of that emotional distress but it's very difficult for me

It's been very tough having MTD for a year now. My whole life changed. I can't socialize like I used to and be there for my loved ones like I used to. I feel very stuck in my life. I cant speak a sentence without my voice cutting of because my pitch changes constantly. The simplest things are the toughest for me like ordering food I stopped doing that it causes me so much anxiety. MTD sure has made me a quiet person . I used to be loud and outgoing. I hope I can be myself again after going to therapies .

Hi there, I have strained my voice doing voiceover work for my job. I am concerned I may have ended my career because I can’t even talk for more than a few words without pain. I am wondering I will ever be able to return to doing voices because I have to put a little strain on them to make different characters. Do you think there is hope? Thank you!!

Absolutely! Vocal pain is not a life sentence. I would recommend having your voice evaluated by a laryngologist. Because you are a professional voice user, you should have good contacts with a laryngologist anyway.

I was diagnosed with MTD a little over two years ago by a team of doctors at Vanderbilt. For those of you unfamiliar with it, it's ironically located in Music City USA (Nashville, TN). They see a ridiculous amount of patients for voice-related issues. My "hardware" was reviewed and everything checked out fine. The recommended treatment was, and still is, botox. Any questions beyond that were, and continue to be, met with, "we don't know why you're experiencing this...it just happens." It's been an incredibly frustrating journey and I dread going anywhere near that hospital or speaking with any of the doctors there. Finding Katarina's KZbin channel provided me with more information (and oddly compassion) than that entire team of doctors. I'm working my way through her videos now and then I'll be able to formulate the next step. Hope to connect with you soon, Katarina, and please continue providing this invaluable service to others.

Hello and thank you for this video. I believe I suffer from this very issue. I stumbled on your video after watching some Shania Twain music videos and, curious about where she’s been lately, noticed one video talking about her voice issues. Oddly enough (or divinely arranged for me to see this), I have suffered with voice issues for nearly two decades. I honestly believe it’s related to having Patulous Eustachian Tube dysfunction and Superior Canal Dehiscence… both, inner ear issues that produce very loud, internal noises. These, in turn, can result in forming very bad habits concerning breathing and talking to try to “quiet” or lesson the internal noises, i.e. loud breathing, loud voicing, loud heartbeat. Fast forward to now, and I KNOW I’m dealing with this as I have many of the sensations you described: tightness, throat tension, feeling of lump, odd vocalization, fatigue in talking…. Side note, I used to be a singer. Unfortunately,not anymore. Do you know of a correlation between the aforementioned ear issues and this? Thank you so much!

I am sorry to hear about your medical and voice issues. I am not aware of any correlations but I believe that everything in our bodies is connected so there is probably some connection. However, I would not self-diagnose because there are other voice issues that can sound or feel the same and be something different. I would recommend visiting a laryngologist who can help with the diagnosis and solutions.

I have been struggling with what I suspect is MTD for 2 years. It was caused after a cold and laryngitis that happened while I also had massive vocal demands at work along with acid reflux. Since that time, I have seen multiple doctors, got 3 laryngoscpies, 1 vocal stroboscopy, and 1 endoscopy. I have addressed the acid reflux by changing my diet, so I am no longer sounding hoarse. My voice sounds clear but there is still alot of pain and tight ess when I speak, especially on the phone but much less in person. I also tried speech language therapy for 4 weeks but it did not help. I am not sure where to go from now. I have no damage to my vocal chords per the multiple scopes I received.

Pain is a complex phenomenon. Reducing vocal pain requires different types of exercises, not physical exercises. They are more in the realm of mindfulness and cognitive strategies, like learning about how human bodies perceive pain and what affects your pain. I’ve struggled with Laryngeal Dystonia for most of my adult life. Almost a year ago it got much worse and My voice has been hoarse since November 2020. Sounds like I have laryngitis. Ive seen 5 specialists including a voice therapist that ruled out all other conditions so I know it’s MTD and it hasn’t gotten better. Not having my voice has made life very difficult. 😢

After a year of good progress in recovery from MTD, a severely psychologically impactful experience, over singing and acid reflux brought my tension back. The tension though is often created due to the permanent pain felt, wether using the voice or not (using voice just makes it worse) that compromises the voice as well as Globus. I was wondering wether you know of any exercises to eliminate globus, and what globus is, and how can we locate where the pain is coming from within the anatomy of the thoat. I was told my vocal folds and larynx are fine but I can’t figure out wether the pain is pharyngeal, or underneath the vocal folds. All I know it the swallowing interferes with the pain.

Globus Pharyngeus (“Globus”) is the medical term for the sensation of a lump in the throat, where no true lump exists. This lump is felt in the adam's apple region of the throat. Hi!! Thanks so much for all your useful videos; I’m so grateful! I have recently been diagnosed with MTD which did not come as a surprise to me. I wonder, should I stop singing freely while I am managing this? I love singing but I’m worried about halting progress if I keep singing using my old harmful ways.

It depends. If singing is really uncomfortable and it hurts, then don't do it for a while. But if you still enjoy singing, try to incorporate techniques that you are using for rebuilding your voice into singing.

I haven’t actually started therapy with a SLP yet as it is a financial investment we need to save up for but rereading my post sounds like I have been doing it; sorry for the confusion. I feel like I use my throat muscles to regulate airflow rather than my diaphragm, especially when attempting higher notes. If I feel strain but still want to sing would humming be ok to protect the larynx? Without a plan from a SLP do you have any advice for me? I’ve watched a bunch of your videos and if there’s a few key ones I should focus on I’d love to know. Also, would your online program be useful to me? Thanks for all you do!

I have been struggling with MTD for about a year now; the ENT told me that there is a lot of tightness in the muscles under my chin and in my neck. Apart from this, I have pretty bad TMJ in my jaw as well as Eustachian Tube Dysfunction, which both create a lot of pressure and tightness around my head. I was wondering, could these be contributing to my neck muscle tightness, and would it be worth doing exercises to loosen my jaw and get rid of sinus pressure? Yes, absolutely! Everything is connected in our bodies so TMJ problems can definitely influence your vocal function. In MTD usually the tension comes from other body parts, not just from your larynx. It manifests in the larynx but the source can be very remote!

I have also been dealing with MTD for more than a year now. A couple of days ago, a friend of mine from India suggested I check out youtube videos on Throat chakra. The first day I listened to it, 5 mins in to the audio, I was actually caught by a surprise, because of the instant relive I felt. My MTD was sever right before that, I had to take 2 weeks off from work because of it. For the first time in a year I was able to go 2 days without my throat hurting. Now whenever i feel the tension coming back i just put my airpods on and listen to. I also put on my airpods over night and listen to it. Hope this help and let me know if it actually works for you. I have been doing this for only 4 days now, will continue to do it and hope for the best.

Yep I've been to E.N.T thought I may have nodules but my throat is all clear, they said it's MTD, I use to sing lovely and clear as me and my brother sing alot of Everly Brothers etc, now I feel like my throat is so tight and I'm sing flat in places..Its crazy even talking my voice just doesn't come out, or its up and down or cracks...aaahhhh

Steve Gribble, I can relate. My singing voice is fine in the upper ranges but my chest voice both singing and speaking is cracking and gruff. I'm waiting on an appointment with a voice specialist, hopefully soon

I understand that it takes time to relearn vocal technique without straining or muscle tension. But how should I think if I have several stage performances this fall, and I probably won´t be able to solve my straining until then? Thank you in advance!

Good question. You do what you can until then, through the period of performances and then again after. You accept yourself and your voice as they are and you work with what you have right now. So, probably you will need more warm up time, more vocal rest time, more hydration, more sleep, avoiding overuse (e.g. skipping after performance parties) and more mindfulness. You accept that tension will be still there when you perform. If you have a functional problem (not structural confirmed by an ENT), then you know that you are not harming your voice by using it.

Thank you for the video! I was diagnosed with mtd and a phonatory gap. But I usually have tension only on one side of my throat, is this possible for mtd? I always have tension on the right side, it goes all the way up to my tounge and soft palate. The other side feels relaxed most of the time, and that's confusing.

Yes, it is confusing how people perceive tension. It's different for everyone. Feeling tension only on one side is more common that feeling it on both sides equally. So, the answer is yes, it is possible.

I have learning from my trainer. Its been a month, i am doing stretches and breathing exercises and next month we will be starting working on voice, i can feel the ease around my throat now but the lump and the pain(little) still feels there on the left side. How can i remove it. Will it be solved in the voice part of the therapy ? Pls tell

Pain and discomfort usually don't go away immediately. It is normal to feel pain and discomfort even though you are doing all the exercises. Be patient and trust the process.

Hello Katarina, I see that you have an online coaching program. Does that program specifically address MTD? The website shows it addresses tension, but doesn't specifically say MTD, so I just wanted to make sure. Also, would it make sense to get diagnosed with MTD (and see if it's primary or secondary) before registering for the program? I've "self-diagnosed" by watching all your videos describing my issue, but I also understand the importance of getting the right professionals to validate any concern. I've seen an ENT once already, who told me he didn't feel anything worrisome when palpating my neck - he gave me a steroid and antiobiotic in case it was a muscle strain or infection; but with no success. The pain/discomfort has actually increased in the last week or so since. I am scheduled to see my PCP and the same ENT again, but they are booked out for a few more weeks - and I just don't want to keep dealing with this discomfort :(

Hi Ree, thank you for your question. Yes, the program is for people who experience vocal tension, strain or pain and most people in the program have the diagnosis of MTD. However, it is not necessary to have the diagnosis. There are other situations that manifest as vocal tension. It really depends on the ENT you visit. Getting a diagnosis of MTD is a long process and sometimes it cannot be confirmed by stroboscopy so many people don't have the official diagnosis but have most likely a condition that falls into that category. I would highly recommend that you speak to a laryngologist (and have your voice evaluated with a stroboscopy) to eliminate other causes. Let me know if you have any other questions.

Is Spasmodic Dysphonia the same as MTD? Could you share videos specific to SD please and ways to help make the voice better. Having SD has made the simplest things so difficult :(

Spasmodic dysphonia is very different from MTD. The cause of SD is different and therefore, some exercises that I am sharing on this channel may be helpful but they will not cure SD. I don't have expertise in SD so I will not post videos on this topic. I am sorry I could not be more helpful.

Raphael, I hear you. Many people with vocal issues are worried about their voice and how it will work throughout the day, next week or in the future. Try to use some mindfulness exercises to calm your body down, for example visualize yourself being successful when giving the presentation.

I have treated with PPI and antibiotics (azithromycin 500mg ) And prednisolone , but this days I don not take any thing except PPI ..

Does anyone know how to overcome MTD? This horrible thing that is so hard for these doctors to diagnose. 3 months ago I woke up with this. I lost my job and my social life from not being able to speak. Anyone who has a success story; how do you over come this? Who helped you? Any advice thank you.

@@healthyvoicetips the doctor that did the Endoscopy saw that my hypopharynx is extremely sensitive.. But the ENT didn’t see anything ‘weird’ … And I’m still suffering with so much pain and problems eating..waking up by choking :(
