Just 5% of patients account for 50% of the costs of healthcare in the United States. Lauran Hardin explains why deeply understanding the causes of chronic illness and the needs of complex-care patients can radically transform outcomes for not only those most at-risk, but for communities as a whole - delivering major reductions in public safety, healthcare, and other costs. Hardin believes that understanding chronic care leads to new opportunities to redesign healthcare for everyone. Lauran Hardin is the Senior Advisor for the Camden Coalition’s National Center for Complex Health and Social Needs. In this role, she builds upon her extensive experience providing consulting, co-design and coaching in care re-design and complex care transformation. Current projects include co-designing a complex care ecosystem for uninsured patients in Memphis, TN, scaling a community based cross-sector care coordination model for vulnerable populations throughout the Adventist Health system and development of a complex care clinic model within the Sutter Health system.
Hardin was previously the Director of a Complex Care Center serving hospitals, multiple providers and more than 1,500 high frequency/complex patients in the Mercy Health System. The Center’s model of complex care has resulted in better patient navigation and outcomes, including decreased emergency room visits, hospitalizations, and costs for vulnerable populations. Aspects of the care model were implemented in more than twenty Trinity Health ministries across six states, including rural and urban locations.
Hardin was awarded the National CNL Vanguard award from the AACN in January 2015. Her innovative model of care earned her “Edge Runner” recognition from the American Academy of Nursing in June of 2015. She was named AARP Culture of Health Scholar at the National Center for Complex Health and Social Needs in January of 2017 and inducted as a Distinguished Fellow of the National Academies of Practice in Nursing in 2018. Hardin earned her Master’s degree in Nursing from the University of Detroit Mercy, with certifications as a CNL, Pain Management and Hospice. She trained as a Facilitator with the Elisabeth Kubler-Ross Center, spent several years working in hospice, and co-developed the first Pain and Palliative Care service in the West Michigan region. I have a chronic illness and there is nothing more soul crushing than feeling like a zombie all the time and being in constant pain, but not being able to get the help you need because nobody believes there's anything wrong with you. If they can't see it, surely it can't be real, right?

It's good to remember that the system is calibrated for "normal" and that if you have a problem that's rare, it's typical to have to keep pushing. This is true in the US and the UK. "Normal" is easier to spot. I am sorry it places a bigger burden on rare illnesses.

I think it was a very good and inspiring talk. It's nice to see people that try to help homeless and outcasts because they are often ignored or resented even though they are also humans and should be treated as such. Also helping the poorest and learning to respect others might really solve a lot of fundamental problems (such as crime rates). Seriously, it's really sad to see so many awful comments under such a good video

That was beautiful......me, I lived in trees, not bushes. Still, these points of action and response are not hypotheses, they are indeed facts. I lived it. Outdoors is how I put it, for a year and a half I lived a lot as Charles did. The concepts Lauren talks about are the same that inspired me to pursue a career in public health. Lying in my bed in that field in Stockton Ca. Thinking......if I had a chance to tell my story, what put me where I was, the state I was in, would it make a difference? Very true thanks for sharing this, social support on all levels is the biggest unmet need in the chronic illness community. SO many people feel alone and only if they knew there were other people out there, better resources, providers to help...things would greatly improve. Autoimmune arthritis is said to be caused by trauma and high stress, among other things. I'm living proof.

If you have 2 choices; 1. Have a perfect health in your whole life, not able to get any decease, no covid, nothing, But you will be poor in your whole life too. 2. You will become a billionaire. Don't have to work in your life again. But you have a chronic illness that make you have to depend on medicine your whole life, experience pain and limited your activities. Which one do you chose? Option 1 or 2 & why?

Well...as a nurse, she should know better as she does have a chronic disease as well. What’s “her story/excuse”? But she is the kind of person that believes a pill, not self restrain, better diet or natural cures, can cure it all (just masks symptoms and makes things worse).

You clearly didn’t hear what she was talking about. You can’t have a better diet if you can’t afford food. If you’re homeless where would you store fruits and vegetables? Chronic illness can’t be wished away. You can’t ignore it away. Addiction, schizophrenia, diabetes, COPD won’t disappear simply because you have a positive attitude. You’re also assuming that the diabetes someone has is caused only by being overweight but that’s not true. This isn’t a pull yourself up by your bootstraps situation.

Natural meds work in the same way other meds work. They mask symptoms. They don’t cure depression, addiction, or even the flu.
