witcher 3 4th ability slot not unlocking

in 2018 i have same problem...two slots will stay locked...very irritating though...

in my char, the top one is the first unlocked mutation slot. the 4th is the last.

have the same problem with (at least) the two first slots. This is the third bug that hinders progress that i found. FIrst one is withe the chest of the wolf armor not spawning. Second was the chest that iniciates the olif armor mision giving random loot but not the objet tht iniciates the quest. Very disapointing with this, Project Red, please... chek your games
