Just getting starting running this campaign (finally!) in Fantasy Grounds. Been looking forward to it, and your videos have given me a good summary of the overall storyline, as well as a few things to look for to tie it together between the adventures. Watching this one, I had a few thoughts: Cohorts can be 2 levels lower than you at the most. There's a feat available to increase that by a level though. Unless you're building it as a combat companion to shore up a weakness in the party, I find cohorts work best to grant you out of combat abilities you might not normally have available. For example, my cohorts usually end up being either item crafters or someone capable of running my stronghold. Maybe a second in command for a mercenary company or something like that. In a campaign that I play in, my cohort is an artificer we can all go to for custom creation requests, and we can send her magic items we don't want so she can break them down for the XP in them (artificer ability). It's easiest in my opinion for the DM to just let them level up behind the scenes to keep them 2 levels behind the party. The extra bookkeeping of dividing experience gains and treasure seems like more trouble than it's worth. In exchange for the guaranteed progression I usually tell the character with the cohort that I'll have some say in how they advance too, mostly to keep the cohort's abilities on par with the party's. That dwarf, though, gave me the impression right away that he's just learning how to be a wizard, so hasn't started bringing down his ASF. The Shielded Casting feat would be really good, and the Abjurant Champion Prestige class would be right up his alley. The ranger could be set up for a swift hunter multiclass with scout if the party is lacking in ranged damage or exploration skills. Especially since scouts get trapfinding like a rogue. The cleric sounds like a good buffbot or healbot that just runs around keeping people standing. The Combat Medic prestige class would be good for him.

I ran a 3.5 campaign with only 3 players and all of the characters had leadership. It worked well

That sounds like an idea. Our Rogue has the Leadership feat. His cohort and his followers are pretty much just background characters that show up from time to time, although his Cohort is always up to something. The Dragon Cohort feat is the cooler Leadership if you ask me. *I acknowledge that the Leadership and Dragon Cohort feats aren't really the same thing.

The fighter wizard really wants a mithral chain shirt or something. Maybe they should have built it as an unarmored dexterity character that uses Mage Armor as you said. 25% Arcane Spell Failure is brutal.
