I've long believed it to be the most difficult standard stroke. Biggest difference of angle of racquet path and target, unusual orientation, sometimes slight variation in grip helps but that just makes it more unique.

Back when Djokovic had Todd Martin as coach, he actually threw the racquet onto his opponents side of the court while serving. If I remember correctly he actually did it twice that season. I think it was 2009 or 2010.
