If the publisher is the author, as most independent authors are there own publisher, would it would be the city where we live or the city where we send all the legal mail regarding the book?

For some reason I thought getting my numbers back would take WAY longer than four days!

Curiously, no. I received two LCCNs in the last month and the longest was 4 days, the other was 2 if I recall.

Ive applied for an LCCN for the same book twice already because I've missed my deadline. And now I'm on my third time. Is this going to be a problem for me?

The problem is that you put in under a publishing company and than said you were independent. That is a contradiction. It would be a business. Also note that if i am not mistaking Amazon will pay the listed publishing company. If that company is you than you need a business account with your name on it. Independent authors would just file under there own name, for their ISBN, as well as their LCCN, and KDP.
